Association of Upper Extremity and Neck Access With Stroke in Endovascular Aortic Repair
Upper extremity and neck access is increasingly being used for complex endovascular aortic repairs. We sought to compare perioperative stroke and other complications of (1) arm/neck with femoral/iliac access (AN) vs femoral/iliac access alone (FI), (2) right- vs left-sided AN, and (3) arm vs neck access sites.

Delphi Study to Reach International Consensus Among Vascular Surgeons on Major Arterial Vascular Surgical Complications
ConclusionVascular surgeons reached international consensus on major and minor complications following AAA, CAD and PAD treatment. This consensus may be helpful in harmonizing the information patients receive and improving standardization of the informed consent procedure. Since major complications differed between diseases, consensus on disease-specific complications to be discussed with patients is necessary.

Cervical Debranching in the Endovascular Era: A Single Centre Experience.
CONCLUSION: The results of cervical debranching procedures showed not only excellent patency rates, but also a significant rate of local complications. Carotid-subclavian bypass appeared to be safer with significantly fewer post-operative complications. Staged hybrid procedures also seemed to be safer. Transapical beating heart mitral valve repair with the NeoChord system: early outcomes of a single-center experience.
Conclusions: Trans-apical off-pump MV repair with the NeoChord system is a safe, minimally invasive procedure, with few minor complications. In well-selected candidates it provides successful treatment of degenerative MR. Results are anatomy dependent, so preoperative patient selection is crucial. PMID: 31119000 [PubMed]

Novel approach to prevention of perioperative stroke in patients undergoing debranching thoracic endovascular aortic repair with mini-cardiopulmonary bypass support
This study was performed to evaluate the efficacy of mini-cardiopulmonary bypass (mini-CPB) support in debranching TEVAR to prevent perioperative stroke.

Outcome After Endovascular Repair of Ruptured Descending Thoracic Aortic Aneurysm: A National Multicentre Study.
CONCLUSIONS: This nationwide multicentre study of patients with rDTAA undergoing TEVAR showed acceptable short- but poor long-term survival. Adequate proximal and distal aortic sealing zones are important for technical success. High risk patients and post-operative complications need to be further addressed in an effort to improve outcome. PMID: 30910493 [PubMed - as supplied by publisher]

A systematic review of patch angioplasty versus primary closure for carotid endarterectomy
Guidelines recommend routine patching after carotid endarterectomy (CEA) on the basis of a lower restenosis rate and presumed lower procedural stroke rate than with primary repair. Underlying evidence is based on studies performed decades ago with perioperative care that significantly differed from current standards. Recent studies raise doubt about routine patching and have suggested that a more selective approach to patch closure (PAC) might be noninferior for procedural safety and long-term stroke prevention.

TAA2. Different Zones (0-2) of Hybrid Debranching Endovascular Aortic Repair: Outcome and Stroke Analysis
The aim of this study was to summarize our single-center experience of hybrid debranching endovascular repair of arch and proximal descending thoracic aorta (DTA) as regards the midterm outcome with highlight of the rate and risk factors of stroke associated with these procedures.

Early Outcomes After Branched and Fenestrated Endovascular Aortic Repair in Octogenarians.
CONCLUSIONS: Complex endovascular repair in octogenarians has higher early all cause mortality compared with non-octogenarians. Rupture and higher ASA class of ≥4 are independent predictors for early mortality. Age ≥80 years was found to be an independent predictor for higher early all cause mortality. Effect of Left Subclavian Artery Revascularisation in Thoracic Endovascular Aortic Repair: A Systematic Review and Meta-analysis.
CONCLUSION: The results of this review reveal that LSA revascularisation was associated with significantly lower peri-operative stroke and SCI rates. LSA revascularisation should be recommended for patients with LSA coverage in TEVAR. High quality RCTs are needed to further validate the conclusion. Timing of Carotid Endarterectomy After Stroke: Retrospective Review of Prospectively Collected National Database
Objective: Our objective was to identify the postoperative risk associated with different timing intervals of repair. Background: Timing of carotid intervention in poststroke patients is widely debated with the scales balanced between increased periprocedural risk and recurrent neurologic event. National database reviews show increased risk to patients treated within the first 2 days of a neurologic event compared to those treated after 6 days. Methods: Utilizing Vascular Quality Initiative data, all carotid interventions performed on stroke patients between the years 2012 and 2017 were queried. Patients were then ...
