Carotid artery stenosis at the time of coronary artery bypass grafting is a risk factor but not a cause for peri-operative stroke
Narayan et al. evaluated whether or not carotid artery screening at the time of coronary artery bypass grafting (CABG) influences neurological outcomes [1]. Two of the four strokes in the group with moderate/severe carotid stenosis occurred on the ipsilateral side of the carotid artery lesion, while two were in areas not related to the side of the carotid artery stenosis. In the group with no significant carotid artery stenosis, 18 patients had a stroke of which 16 were ischemic and 2 were hemorrhagic [1].

Comparison of Trends and In-Hospital Outcomes of Concurrent Carotid Artery Revascularization and Coronary Artery Bypass Graft Surgery: The United States Experience 2004 to 2012
Conclusions In patients with concomitant carotid and coronary disease undergoing combined revascularization, combined CEA+CABG is utilized most frequently, followed by staged CEA+CABG and staged CAS+CABG strategies. The staged CAS+CABG strategy was associated with lower risk of mortality, but higher risk of stroke. Future studies are needed to examine the risks/benefits of different carotid revascularization strategies for high-risk patients requiring concurrent CABG.

Influence of unilateral, asymptomatic severe carotid artery stenosis in patients undergoing coronary artery bypass grafting.
CONCLUSIONS: 1.Asymptomatic, unilateral severe carotid artery stenosis does not increase the risk of stroke, myocardial infarction and mortality within 30 days of coronary artery bypass grafting. 2.The presence of asymptomatic unilateral, severe carotid artery stenosis increases the risk of cardiovascular events during the first 12 months postoperatively.
