The Top 7 Tox Articles of 2013
As we celebrate the year just ending, and welcome in 2014, let’s pause and remember some of the articles in the field of medical toxicology that most impressed TPR during 2013. The following 7 papers are listed in chronological order (links connect to TPR‘s discussion of each article): Pediatric Marijuana Exposures in a Medical Marijuana State. Wang GS et al. JAMA Pediatr 2013 July;167:630-3 With several states legalizing recreational marijuana — and many more allowing medical marijuana — cases of inadvertent pediatric exposure to cannabis will undoubtedly increase, and it is important for emergency practi...

Model-predicted occurrence of multiple pharmaceuticals in Swedish surface waters and their flushing to the Baltic Sea.
Abstract An exposure assessment for multiple pharmaceuticals in Swedish surface waters was made using the STREAM-EU model. Results indicate that Metformin (27 ton/y), Paracetamol (6.9 ton/y) and Ibuprofen (2.33 ton/y) were the drugs with higher amounts reaching the Baltic Sea in 2011. 35 of the studied substances had more than 1 kg/y of predicted flush to the sea. Exposure potential given by the ratio amount of the drug exported to the sea/amount emitted to the environment was higher than 50% for 7 drugs (Piperacillin, Lorazepam, Metformin, Hydroxycarbamide, Hydrochlorothiazide, Furosemide and Cetirizine), implyi...

Long‐term effects of metformin on endothelial function in type 2 diabetes: a randomized controlled trial
ConclusionsMetformin is associated with improvement in some (vWF and sVCAM‐1) but not all markers of endothelial function, which may explain why it is associated with a decreased risk of CV disease in type 2 diabetes. This article is protected by copyright. All rights reserved.

Metformin for pediatric obesity and insulin resistance: a retrospective study within an integrated health care system
CONCLUSIONS: Metformin with lifestyle interventions significantly reduced weight, BMI, and BMI z score in pediatric patients with obesity and insulin resistance up to 24 months, compared with intensive and routine counseling alone.PMID:34355850 | DOI:10.1002/oby.23207

Diabetes Drug Prescription Pattern and Awareness Among Health Care Providers in Sub-Himalayan Region of India: A Population Based Study.
CONCLUSIONS: Sulphonylureas were the commonest anti-diabetic drug prescribed by the primary care providers followed by metformin. Insulin was prescribed to 2.23% only. Combination of amlodipine and atenolol was the commonest anti-hypertensive drugs prescribed and only 10% of patients were prescribed statin. PMID: 28598049 [PubMed - in process]

MET-PREVENT: metformin to improve physical performance in older people with sarcopenia and physical prefrailty/frailty - protocol for a double-blind, randomised controlled proof-of-concept trial
Introduction Skeletal muscle dysfunction is central to both sarcopenia and physical frailty, which are associated with a wide range of adverse outcomes including falls and fractures, longer hospital stays, dependency and the need for care. Resistance training may prevent and treat sarcopenia and physical frailty, but not everyone can or wants to exercise. Finding alternatives is critical to alleviate the burden of adverse outcomes associated with sarcopenia and physical frailty. This trial will provide proof-of-concept evidence as to whether metformin can improve physical performance in older people with sarcopenia and phy...

Management of diabetic medications prior to FDG-PET/CT: Assessment of nationwide approach to exam preparation.
Conclusion: As per the ACR guidelines, most centers have a preparation protocol for diabetic patients undergoing FDG-PET/CT, including thresholds for hyperglycemia, and control of insulin administration prior to exam. However, despite published literature on metformin-induced FDG bowel uptake, few centers are focusing on metformin discontinuation prior to scanning. Very few centers rely on radiologist review of diabetic medications prior to exam scheduling. In view of the variability of practice around the nation, a standardized, evidence-based approach should be formulated to improve patient care. Research Support: None

Metformin counteracts glucose-dependent lipogenesis and impairs transdeamination in the liver of gilthead sea bream (Sparus aurata).
Abstract Metformin is an anti-diabetic drug with a major impact on regulating blood glucose levels by decreasing hepatic gluconeogenesis but also affecting other pathways, including glucose transport and energy/lipid metabolism. Carnivorous fish are considered glucose intolerant, as they exhibit poor ability to using dietary carbohydrates. To increase the current knowledge about the molecular mechanisms by which metformin can improve glucose homeostasis in carnivorous fish, we addressed the effect of intraperitoneal administration of metformin, in the presence or absence of a glucose load, on metabolic rate-limiti...
