Low-dose aspirin in the prevention of pre-eclampsia in China: postpartum hemorrhage in subgroups of women according to their characteristics and potential bleeding risk
CONCLUSIONS: A dosage of 100 mg of aspirin per day, initiated from 12 to 20 gestational weeks until 34 weeks of gestation, did not increase the risk of potential bleeding and PPH regardless of the maternal characteristic. In the aspirin group, the positive correlation between BMI and PPH was significant.TRIAL REGISTRATION: ClinicalTrials.gov, NCT01979627.PMID:36914957 | DOI:10.1097/CM9.0000000000002545

Changes in process and outcome for ST elevation myocardial infarction in central China from 2011 to 2018
CONCLUSIONS: Gradual implementation of the guideline-recommended treatments in STEMI patients from 2011 to 2018 has been associated with decreased in-hospital mortality. However, gaps persist between clinical practice and guideline recommendation. Public awareness, reperfusion strategies, and construction of chest pain centers need to be further underscored in central China.PMID:37545028 | DOI:10.1097/CM9.0000000000002698

Optimal medical treatment versus carotid endarterectomy: the rationale and design of the Aggressive Medical Treatment Evaluation for Asymptomatic Carotid Artery Stenosis (AMTEC) study
The objective is to compare the efficacy of carotid endarterectomy + optimal medical therapy versus optimal medical therapy alone in patients with asymptomatic (70–79%) extracranial carotid stenosis. DesignThe Aggressive Medical Treatment Evaluation for Asymptomatic Carotid Artery Stenosis study is a prospective, randomized, parallel, two‐arm, multicenter trial. Primary end‐points will be analyzed using standard time‐to‐event statistical modeling with adjustment for major baseline covariates. The primary analysis is on an intent‐to‐treat basis. Study OutcomesThe primary outcome is nonfatal stroke, nonfata...

Current medical management of stable coronary artery disease before and after elective percutaneous coronary intervention
Background: Percutaneous coronary intervention (PCI) for stable coronary artery disease (CAD) is not superior to optimal medical therapy. It remains unclear if patients who receive PCI for stable CAD are receiving appropriate medical therapy. Methods: We evaluated the medical management of 60,386 patients who underwent PCI for stable CAD between 2004 and 2009. We excluded patients with contraindications to aspirin, clopidogrel, statins, or β-blockers (BBs). We defined essential medical therapy of stable CAD as treatment with aspirin, statin, and BB before PCI and treatment with aspirin, clopidogrel, and statin after PCI.Re...

Hip replacement deaths drop by a half since 2003
Conclusion This is an informative study which demonstrates a decline in 90-day mortality rates following total hip replacement in England and Wales between 2003 and 2011. The study benefits from using reliable datasets: the National Joint Registry for England and Wales is said to have recorded all total hip replacements performed since 2003; the Office for National Statistics records all deaths; and the Hospital Episode Statistics records data for all people who have received NHS-funded hospital care. The research has identified factors associated with decreased or increased 90-day mortality, although it is not possible to...

Efficacy of contemporary medical management for asymptomatic carotid artery stenosis.
Abstract In the Asymptomatic Carotid Artery Stenosis trial (1995), medical management was defined as aspirin in addition to adequate control of comorbidities. Since then, medical management of asymptomatic carotid artery stenosis (CAS) has progressed to include broader use of statins. Our purpose was to review the effect of contemporary medical management on stroke prevention. A retrospective review of the Kaiser Permanente, Southern California medical group database was performed. All patients with a diagnosis of asymptomatic CAS by International Classification of Diseases, 9th Revision codes from 2007 to 2011 we...

Comparison between Endoscopic Therapy and Medical Therapy in Peptic Ulcer Patients with Adherent Clot: A Multicenter Prospective Observational Cohort Study.
CONCLUSIONS: In patients with Forrest type IIB peptic ulcer bleeding, endoscopic therapy was associated with a significant reduction in bleeding related mortality and all cause mortality compared with medical therapy alone. Important risk factor of rebleeding was use of aspirin and/or NSAID. PMID: 26289243 [PubMed - in process]

Medication therapy in ambulatory medical care: United States, 2003-04.
This report describes medication therapy at visits to physician offices, hospital outpatient departments, and emergency departments in the United States during 2003 and 2004. Office-based care is further subdivided into three categories-primary care, surgical specialties, and medical specialties. METHODS: Data from the 2003 and 2004 National Ambulatory Medical Care Surveys (NAMCS) and National Hospital Ambulatory Medical Care Surveys (NHAMCS) were combined to produce averaged annual estimates of ambulatory medical care utilization. RESULTS: An estimated 1.9 billion drugs per year were provided, prescribed, or conti...

Subacute stent thrombosis in a patient with COVID-19 pneumonia
We present a case of a patient with COVID-19 infection who presented with pneumonia, acute respiratory failure, and non-ST elevation myocardial infarction. Reperfusion was achieved with coronary angioplasty and drug-eluting stent placement, and the patient was started on aspirin and clopidogrel. However, 72 hours later, the patient experienced an episode of subacute stent thrombosis. This raises concerns about a possible association between stent thrombosis and increased thrombogenicity and platelet aggregability triggered by COVID-19 infection.PMID:33456191 | PMC:PMC7785200 | DOI:10.1080/08998280.2020.1812792
