CHIVA method for the treatment of chronic venous insufficiency.
CONCLUSIONS: The CHIVA method reduces recurrence of varicose veins and produces fewer side effects than vein stripping. However, we based these conclusions on a small number of trials with a high risk of bias as the effects of surgery could not be concealed. New RCTs are needed to confirm these results and to compare CHIVA with approaches other than open surgery. PMID: 23821413 [PubMed - as supplied by publisher]

Anticoagulation for the initial treatment of venous thromboembolism in patients with cancer.
CONCLUSIONS: LMWH is possibly superior to UFH in the initial treatment of VTE in patients with cancer. Additional trials focusing on patient-important outcomes will further inform the questions addressed in this review. PMID: 24945634 [PubMed - as supplied by publisher]

Preoperative vascular access evaluation for haemodialysis patients.
CONCLUSIONS: Based on four small studies, preoperative vessel imaging did not improve fistula outcomes compared with standard care. Adequately powered prospective studies are required to fully answer this question. PMID: 26418347 [PubMed - as supplied by publisher]

Early versus delayed erythropoietin for the anaemia of end-stage kidney disease.
CONCLUSIONS: We found no evidence to assess the benefits and harms of early versus delayed EPO for the anaemia of ESKD. PMID: 26671531 [PubMed - as supplied by publisher]

Anticoagulants and antiplatelet agents for preventing central venous haemodialysis catheter malfunction in patients with end-stage kidney disease.
CONCLUSIONS: The relative net benefit of anticoagulant therapies for prevention of catheter malfunction remains uncertain. Multiple agents appear to reduce catheter-related bacteraemia although the lack of clear assessment of harms and the limitations of study quality mean these results should be interpreted with caution. Methodological approaches can be used to avoid methods of reporting unduly affecting on the results of meta-analyses incorporating studies employed mixed reporting methods. Further high quality randomised studies, including safety outcomes, are needed. PMID: 27039404 [PubMed - as supplied by publisher]

Cancers, Vol. 14, Pages 1937: In Search of the Appropriate Anticoagulant-Associated Bleeding Risk Assessment Model for Cancer-Associated Thrombosis Patients
sabelle Mahé Patients with venous thromboembolism events (VTE) in the context of cancer should receive anticoagulants as long as the cancer is active. Therefore, a tailor-made anticoagulation strategy should rely on an individualized risk assessment model (RAM) of recurrent VTE and anticoagulant-associated bleeding. The aim of this review is to investigate the applicability of the currently available RAMs for anticoagulant-associated bleeding after VTE in the CAT population and to provide new insights on how we can succeed in developing a new anticoagulant-associated bleeding RAM for the current medical care of CAT pa...

Fibrinogen: A Circulating Factor in Search for Its Genetic Architecture.
Abstract Fibrinogen (Coagulation factor I) is a major player in thrombus formation; it is cleaved by thrombin to form fibrin, which is the most abundant component of a blood clot(1). Beyond the role played in the coagulation and cardiovascular diseases (CVD), fibrinogen is a proinflammatory factor in autoimmune and inflammatory diseases (such as rheumatoid arthritis, vasculitides, inflammatory bowel disease, multiple sclerosis, chronic obstructive pulmonary diseases and kidney disorders and post-transplant-fibrosis)(2) as well as in several types of cancer(3). Fibrinogen has been demonstrated to interfere with the...

Feasibility and cost effectiveness of stroke prevention through community screening for atrial fibrillation using iPhone ECG in pharmacies. The SEARCH-AF study.
Abstract Atrial fibrillation (AF) causes a third of all strokes, but often goes undetected before stroke. Identification of unknown AF in the community and subsequent anti-thrombotic treatment could reduce stroke burden. We investigated community screening for unknown AF using an iPhone electrocardiogram (iECG) in pharmacies, and determined the cost-effectiveness of this strategy.Pharmacists performedpulse palpation and iECG recordings, with cardiologist iECG over-reading. General practitioner review/12-lead ECG was facilitated for suspected new AF. An automated AF algorithm was retrospectively applied to collecte...

Finite Element Analysis in Asymptomatic, Symptomatic, and Ruptured Abdominal Aortic Aneurysms: In Search of New Rupture Risk Predictors.
CONCLUSIONS: From different FEA derived parameters, PWRI distinguishes most precisely between asymptomatic and symptomatic AAA. If elevated, this value may represent a negative prognostic factor for asymptomatic AAAs. PMID: 25542592 [PubMed - as supplied by publisher]

Deep Vein Thrombosis Prophylaxis in Patients with Traumatic Brain Injury.
CONCLUSION: Head trauma alone is a risk factor for deep vein thrombosis and pulmonary thromboembolism and the risks inherent in this disease requires methods of prevention for these complications. Clinical trials are needed to establish the efficacy of prophylaxis and the best time to start medication for deep vein thrombosis in patients with traumatic brain injury. CCC Update 011
Updates to the LITFL Critical Care Compendium are coming thick and fast at the moment. Here’s a quick summary as these entries don’t appear in the LITFL blog feed, and you otherwise need to search for them using search keywords in the CCC table, by googling “litfl ccc searchterm” or by searching GoogleFOAM. Remember, constructive feedback is the fuel that LITFL runs on, hit us with it! Here’s what is new in the CCC since CCC Update 010: Cerebral venous thrombosis This condition reared its head in the last round of FCICM exam vivas I believe — and it wouldn’t surprise me if it...
