[COVID-19 and Seasonal Flu During the Autumn-Winter of 2020/2021 and the Challenges Lying Ahead for Hospitals].
Authors: Campos L, Mansinho K, Telles de Freitas P, Ramos V, Sakellarides C Abstract The possibility of a second wave of the COVID-19 pandemic coexisting with a simultaneous epidemic of influenza and the co-circulation of other seasonal respiratory viruses sets the stage for a perfect storm. Preparing for the Autumn-Winter of 2020/2021 is complex, requiring centralized guidance but local and regional solutions, with strong leadership and a high level of coordination. It is essential to act upstream of hospitals in order to reduce demand on emergency departments, minimizing the risk of transmission that occurs there...

Diagnosis and Treatment of Neuroleptic Malignant Syndrome in the Intensive Care Unit: A Case Report.
We present the case of a 66-year-old man with severe neuroleptic malignant syndrome, diagnosed due to need for continuous invasive ventilation in an Intensive Care Unit, after successful treatment for respiratory sepsis. The patient recovered after electroconvulsive therapy and administration of bromocriptine. This unusually severe case illustrates the need for a high level of suspicion for neuroleptic malignant syndrome in critically ill patients with malignant catatonic syndromes, allowing for an early diagnosis and potentially lifesavingtreatment. Lithium Intoxication after Bariatric Surgery: A Case Report.
Authors: Marques AR, Alho A, Martins JM, Bairrão M Abstract Bariatric surgery is a therapeutic option to treat obesity in (carefully selected) patients with psychiatric disorders. About half of the patients referred for bariatric surgery have a diagnosis of (at least one) mental disorder and most of them are treated with psychotropic drugs. This procedure may modify the bioavailability of drugs and lithium is no exception. However, although absorption seems to decrease in most drugs, in the case of lithium, there is a high risk of toxicity. In this article, we describe the case of a 44-year-old female patien...

[Urinary Retention Due to Hematocolpos].
Authors: Silva ÍS, Martello R, Mendes A, Chaves A Abstract Hematocolpos is an obstruction of the menstrual flow due to an anomaly of the genital tract, with imperforate hymen being the most common one. It is an uncommon condition, with a prevalence of 1:1000 - 1:16 000. It is usually an asymptomatic condition until the onset of menarche, when there is accumulation of blood in the vagina (hematocolpos) or in the uterus (hematometra). A rare symptom of hematocolpos could be urinary retention. Our case report is about a 12-year-old girl, with no menarche and Tanner stage M4/P5, who was admitted to our emergency...

Strongyloidiasis: A Diagnosis to Consider in Previously Endemic Regions in Portugal.
We report the case of a patient with abdominal pain, ascites and marked eosinophilia. The diagnostic investigation excluded frequent causes of eosinophilia. The diagnosis of strongyloidiasis was confirmed by immunoenzymatic positivity of the antibody to Strongyloides stercoralis. Since the patient lived in a region where several cases of strongyloidiasis were diagnosed in the past, and given the absence of previous trips abroad, this is the first case identified in recent decades of autochthonous strongyloidiasis in Portugal. The authors point out the possibility of chronic infection acquired in regions previously consider...

Jejunojejunal Intussusception After Roux-en-Y Gastric Bypass and Abdominoplasty: A Rare Association or a Mere Coincidence?
We report the case of a 42-year-old woman, with an history of a laparoscopic Rouxen-Y gastric bypass, who developed small bowel occlusion secondary to a retrograde jejunojejunal intussusception in the second post-operative day of an abdominoplasty. This is the first report of a small bowel intussusception after Roux-en-Y gastric bypass in the post-operative period of an abdominoplasty. Doubts remain about the possible causal effect abdominoplasty might have in small bowel intussusception after Roux-en-Y gastric bypass. Estimation of the 10-Year Risk of Fatal Cardiovascular Disease in the Portuguese Population: Results from the First Portuguese Health Examination Survey (INSEF 2015).
CONCLUSION: In 2015, a considerable proportion of the Portuguese population aged between 40 and 65 years old had a high or very high risk of developing a fatal cardiovascular event in the next 10 years. Due to the possible overestimation of the cardiovascular risk already reported in other European countries, it will be important to carry out a follow-up study to validate the adequacy of using the SCORE algorithm in the Portuguese population.
