Regional Anesthesia is Associated With Decreased Postoperative Hypertension in Patients Undergoing Eversion Carotid Endarterectomy
Eversion carotid endarterectomy (eCEA) has been effective in reducing stroke risk in patients with carotid artery stenosis. Recent literature shows that patients who undergo eCEA are at risk for development of postoperative hypertension requiring intravenous (IV) antihypertensive medication. The purpose of this study was to assess whether the type of anesthesia (regional or general) has an impact on the incidence of postoperative hypertension requiring IV antihypertensive medication in the surgical intensive care unit (SICU) in patients undergoing eCEA.

Long-term consequences of acute kidney injury in the perioperative setting
Purpose of review: Recent studies indicate that acute kidney injury (AKI) and chronic kidney disease (CKD) are interconnected syndromes. Although the majority of patients who suffer an episode of AKI will recover laboratory indices suggesting complete or near complete recovery of renal function, a significant portion of post-AKI survivors will develop major kidney events, including development of late-stage CKD, need for renal replacement therapies, and death. Recent findings: Our review highlights epidemiology of adverse post-AKI events, association of AKI with late development of nonrenal adverse outcomes, use of bedsid...

Delirium after cardiac surgery: A pilot study from a single tertiary referral center.
CONCLUSIONS: Patients who developed delirium had systemic disease in the form of hypertension and cerebrovascular disease. Delirium was seen in patients who had higher postoperative pain scores, longer ICU stay, and NIV use. This study can be used to develop a predictive tool for diagnosing postcardiac surgical delirium. Pulmonary hypertension attenuates the dynamic preload indicators increase during experimental hypovolemia
Pulse pressure (PPV) and stroke volume (SVV) variations may not be reliable in the setting of pulmonary hypertension and/or right ventricular (RV) failure. We hypothesized that RV afterload increase attenuates...

Ketamine Therapy for Treatment-resistant Depression in a Patient with Multiple Sclerosis: A Case Report
Conclusion—Ketamine may be an alternative treatment for resistant depression and may have a special use in patients with multiple sclerosis. Introduction Depression is a frequent finding in patients with multiple sclerosis (MS), with the lifetime prevalence rates for major depressive disorder (MDD) ranging from 36 to 54 percent, more than twice of that in the general population.[1] Even with advances in pharmacological options for treating depression, an estimated 33 to 66 percent of patients with MDD in the general population do not respond to the first antidepressant, and a reported 15 to 33 percent of patients do not ...

Committee Opinion No. 692: Emergent Therapy for Acute-Onset, Severe Hypertension During Pregnancy and the Postpartum Period.
Authors: Abstract Acute-onset, severe systolic hypertension; severe diastolic hypertension; or both can occur during the prenatal, intrapartum, or postpartum periods. Pregnant women or women in the postpartum period with acute-onset, severe systolic hypertension; severe diastolic hypertension; or both require urgent antihypertensive therapy. Introducing standardized, evidence-based clinical guidelines for the management of patients with preeclampsia and eclampsia has been demonstrated to reduce the incidence of adverse maternal outcomes. Individuals and institutions should have mechanisms in place to initiate the...

Clinical ‘pearls’ of maternal critical care Part 2: sickle-cell disease in pregnancy
Purpose of review: The current review outlines the challenges in managing pregnant women with sickle-cell anemia, who are at risk of becoming critically ill during pregnancy. Recent findings: Sickle obstetric patients pose unique challenges to the anesthetist and intensivist. We discuss the role of prophylactic transfusions for specific indications like acute anemia and twin pregnancies. The management and prevention of vaso-occlusive crises and chest crisis are also outlined. The role of the multidisciplinary team cannot be overstated. Massive obstetric hemorrhage in this population is difficult, and unique consideratio...

Severe Maternal Morbidity in a Large Cohort of Women With Acute Severe Intrapartum Hypertension
(Am J Obstet Gynecol. 2016;215(1):91.e1–91.e7) Adverse effects of hypertensive disorders of pregnancy such as stroke, eclampsia, hemolysis, elevated liver enzymes, low platelet count syndrome, renal failure, and disseminated intravascular coagulation are commonly associated with high rates of maternal morbidity and mortality in the United States. The present study aimed to analyze existing management patterns in a large group of women with severe intrapartum hypertension, to determine the maternal morbidity associated with severe intrapartum hypertension and compare it to that in women without severe hypertension.

Systemic hypertension and non-cardiac surgery
Satyajeet MisraIndian Journal of Anaesthesia 2017 61(9):697-704Primary systemic hypertension affects 10%–25% of individuals presenting for surgery and anaesthesia and constitutes an important cause of cancellation of elective surgeries. Much of the fear stems from the fact that hypertension may lead to adverse perioperative outcomes. Although long-standing hypertension increases the risk of stroke, renal dysfunction or major adverse cardiovascular events, the same is usually not seen in the perioperative period if blood pressure is <180/110 mmHg and this has been the overriding...

Abstract P421: The Effects of Dual Angiotensin Receptor and Neprilysin Inhibitor on Organoprotection in Experimental Model of Chronic Heart Failure Session Title: Renin-Angiotensin System
The cardiac effects of dual angiotensin receptor and neprilysin inhibitor (LCZ696) as a novel therapeutic approach for inhibition of renin-angiotensin system was assessed in volume overload-induced chronic heart failure (CHF) in hypertensive Ren2 transgenic rats (TGR) with aorto-caval fistula (ACF) and compared with valsartan (VAL). CHF model was induced by ACF in male TGR at 8 weeks of age. After 5 weeks of ACF induction, LCZ696 was administered in standard dosing at 68 mg/kg/day and VAL at 31 mg/kg/day for next 15 weeks to assess the effect on mortality. In second series, 12 weeks after ACF, the animals were anaesthetize...

Right Ventricular Perfusion Physiology and Clinical Implications
Regulation of blood flow to the right ventricle differs significantly from that to the left ventricle. The right ventricle develops a lower systolic pressure than the left ventricle, resulting in reduced extravascular compressive forces and myocardial oxygen demand. Right ventricular perfusion has eight major characteristics that distinguish it from left ventricular perfusion: (1) appreciable perfusion throughout the entire cardiac cycle; (2) reduced myocardial oxygen uptake, blood flow, and oxygen extraction; (3) an oxygen extraction reserve that can be recruited to at least partially offset a reduction in coronary blood ...

Unknown ‐Onset Strokes with Anterior Circulation Occlusion treated by Thrombectomy after DWI‐FLAIR Mismatch Selection
ConclusionThrombectomy of UOS with anterior circulation occlusion and DWI‐FLAIR mismatch appears to be as safe and efficient as thrombectomy of KOS within 6 hours from onset. This pattern of imaging could be used for patient selection when time of onset is unknown. This article is protected by copyright. All rights reserved.
