Intravenous thrombolysis for patients with in ‐hospital stroke onset: propensity‐matched analysis from the SITS‐EAST registry
ConclusionsOur findings underline the safety and efficacy of IVT for IHS. They also underscore the potential of reducing in‐hospital delays for timely tPA delivery in patients with IHS.This article is protected by copyright. All rights reserved.

Safety and effectiveness of CATCH+ as a first-line device for revascularization in the treatment of acute ischemic stroke
Conclusions: The CATCH+ device is a safe and effective mechanical thrombectomy device for the first-line treatment of acute ischemic stroke.

Mechanical Thrombectomy in Acute Ischemic Stroke Using a  Manually Expandable Stent Retriever (Tigertriever)
ConclusionThe Tigertriever offers a  safe and effective treatment option in ischemic stroke due to LVOs with reperfusion rates and a safety profile similar to alternative devices. The Tigertriever is a promising bail-out tool in complex cases. Its role as a first line device has to be evaluated in further prospective studies.

Use of intracranial stent as rescue therapy after mechanical thrombectomy failure —9-year experience in a comprehensive stroke centre
ConclusionPlacement of an intracranial stent as rescue therapy after MT failure was associated with better clinical outcome without significant increase in haemorrhagic complications. We believe that this procedure should be appraised in these patients.

Combined therapy of intensive statin plus intravenous rt-PA in acute ischemic stroke: the INSPIRE randomized clinical trial
ConclusionThe INSPIRE study indicated that intensive statin therapy may not improve clinical outcomes compared with the low dose of statin therapy in AIS patients undergoing intravenous thrombolysis, and the two groups had similar safety profile.Clinical trial registrationURL:http://www.chictr.org. Functional and technical outcomes in acute ischemic stroke patients with hyperdense middle cerebral artery sign treated with endovascular thrombectomy
ConclusionPatients with HMCAS treated with EVT do not have a worse outcome at 3  months compared with no-HMCAS patients. Patients with HMCAS required a greater number of thrombus passes and longer procedure times.

Computed tomography perfusion-based thrombolysis in wake-up stroke
Abstract Wake-up stroke (WUS) patients are typically excluded from reperfusion treatment, as the time of symptoms onset is unknown. The purpose of this study is to evaluate the clinical outcome and safety of intravenous thrombolysis with rt-PA in patients with WUS eligible for therapy using computed tomography perfusion criteria (CTP), compared to patients treated with rt-PA within 4.5 h of symptoms onset (non-WUS). This is an experimental, open-label trial, controlled against the best therapy currently in use. Primary endpoints were functional independence after 3 months [modified Rankin scale (mRS) ≤ 1] ...

Stereotactic Catheter Ventriculocisternostomy for Clearance of Subarachnoid Hemorrhage Clinical Sciences
Conclusions—STX-VCS was feasible and safe in patients with severe aneurysmal subarachnoid hemorrhage. Initial results indicate that DCI and mortality can be reduced, and neurological outcome may be improved with this method.

Predictors of favorable outcome after mechanical thrombectomy for anterior circulation acute ischemic stroke in octogenarians
Conclusions There is no reason to withhold mechanical thrombectomy on the basis of age alone. Small infarct core, low NIHSS, male gender and use of IVT are associated with a better functional outcome.

Development of a decision analytic model to support decision making and risk communication about thrombolytic treatment
Conclusions: We have produced a DAM that provides an estimation of the likely benefits and risks of thrombolysis for individual patients, which has subsequently been embedded in a computerised decision aid to support better decision-making and informed consent.

“A contemporary description of staphylococcus aureus prosthetic valve endocarditis. Differences according to the time elapsed from surgery”
Staphylococcus aureus prosthetic valve endocarditis (SAPVE) has a poor prognosis. There are no large series that accurately describe this entity. This is a retrospective observational study on a prospective cohort from 3 Spanish reference hospitals for cardiac surgery, including 78 definitive episodes of left SAPVE between 1996 and 2016. Fifty percent had a Charlson Index score>5; 53% were health care-related. Twenty percent did not present fever. Complications at diagnosis included: severe heart failure (HF, 29%), septic shock (SS, 17.9%), central nervous system abnormalities (19%), septic metastasis (4%). Hemorrhagic s...

Thrombectomy or Intravenous Thrombolysis in patients with NIHSS of 5 or less?
Conclusions: MT in patients with minor strokes and intracranial vessel occlusion (IVO) is safe and can determine a rapid improvement of NIHSS score. MT seems also associated with a higher rate of patients discharged at home after hospitalization and better clinical outcome at 3-month follow-up. Larger randomized trials are warranted to confirm these results.

Diffuse cerebral petechial hemorrhage in an 8-year-old girl with MRSA pneumonia and sepsis
An 8-year-old girl in septic shock due to necrotizing methicillin-resistant Staphylococcus aureus (MRSA) pneumonia developed signs of end-organ damage, new right hemiplegia, and left gaze preference. Susceptibility-weighted MRI demonstrated extensive multifocal petechial hemorrhage preferentially at the gray–white matter interface due to septic microemboli (figure). Mechanisms of hemorrhage include small-vessel occlusion leading to mycotic aneurysm formation with rupture or pyogenic arteritis without aneurysm formation.1 Intracranial hemorrhage associated with metastatic staphylococcal infections is rarely descr...
