Sleep apnea and risk of vertigo: A nationwide population ‐based cohort study
ConclusionThis is the first nationwide population‐based cohort study to investigate the association between sleep apnea and vertigo. The findings strongly support that sleep apnea is an independent risk factor for vertigo. Based on the study results, physicians should be aware of potential vertigo occurrence following sleep apnea. The Association of Arsenic Metabolism with Cancer, Cardiovascular Disease, and Diabetes: A Systematic Review of the Epidemiological Evidence
Conclusions: Population level of iAs% and DMA%, but not MMA%, were associated with arsenic exposure levels. Overall, study findings suggest that higher MMA% was associated with an increased risk of cancer and cardiovascular disease, while lower MMA% was associated with an increased risk of diabetes and metabolic syndrome. Additional population-based studies and experimental studies are needed to further evaluate and understand the role of arsenic exposure in arsenic metabolism and the role of arsenic metabolism in disease development. https://doi.org/10.1289/EHP577 Received: 01 June 2016 Revised: 26 February 2017 Acce...

Previously undiagnosed risk factors and medication nonadherence are prevalent in young adults with first ‐ever stroke
ConclusionsPreviously undiagnosed hypertension and nonadherence to treatment of hypertension and diabetes were more prevalent in young adult patients with first‐ever stroke in Taiwan. Interventions targeting young people to promote early detection and adequate control of VRFs should be encouraged.

2017 Taiwan lipid guidelines for high risk patients.
Abstract In Taiwan, the prevalence of hyperlipidemia increased due to lifestyle and dietary habit changes. Low density lipoprotein cholesterol (LDL-C) and non-high density lipoprotein cholesterol (non-HDL-C) are all significant predicting factors of coronary artery disease in Taiwan. We recognized that lipid control is especially important in patients with existed atherosclerotic cardiovascular diseases (ASCVD), including coronary artery disease (CAD), ischemic stroke and peripheral arterial disease (PAD). Because the risk of ASCVD is high in patients with diabetes mellitus (DM), chronic kidney disease (CKD) and f...

Perioperative medicine and Taiwan National Health Insurance Research Database.
Abstract "Big data", characterized by 'volume', 'velocity', 'variety', and 'veracity', being routinely collected in huge amounts of clinical and administrative healthcare-related data are becoming common and generating promising viewpoints for a better understanding of the complexity for medical situations. Taiwan National Health Insurance Research Database (NHIRD), one of large and comprehensive nationwide population reimbursement databases in the world, provides the strength of sample size avoiding selection and participation bias. Abundant with the demographics, clinical diagnoses, and capable of linking divers...

No increased risk of coronary heart disease for patients receiving androgen deprivation therapy for prostate cancer in Chinese/Taiwanese men
This study included 1278 patients with prostate cancer in the study group and 1278 subjects without prostate cancer in the comparison group. Each patient was individually tracked for a 3‐year period to identify those who had subsequently received a diagnosis of CHD. The results showed that the incidence rate of CHD during the 3‐year follow‐up period was 4.69 (95% CI: 2.99–5.48) per 100 person‐years and 2.67 (95% CI: 2.15–3.27) per 100 person‐years for the study and comparison cohort, respectively. The Cox proportional hazard regression showed that the hazard ratio for CHD during the 3‐year follow‐up perio...

Guideline of neuropathic pain treatment and dilemma from neurological point of view.
Authors: Yang CM, Chen NC, Shen HC, Chou CH, Yeh PS, Lin HJ, Chang CY, Cheng TJ, Lin KC Abstract Neuropathic pain is a complicated symptomatic disease as migraine in recent years. Not because the pain character differed from the nociceptive inflammatory symptoms but because of its complexity of mechanisms. Though peripheral sensitization, ectopic discharge, central sensitization, central re-organization and loss of inhibition play part of roles in mechanisms, however, based on this mechanistic treatment, the outcome still disappointed physicians and patients, exampled as central post-stroke central pain (CPSP). The...

Post-traumatic stress disorder was associated with a threefold increased risk for stroke in a Taiwanese National Health Insurance Database
ABSTRACT FROM: Chen MH, Pan TL, Li CT, et al. Risk of stroke among patients with post-traumatic stress disorder: nationwide longitudinal study. Br J Psychiatry 2015;206:302–7. What is already known on this topic People with post-traumatic stress disorder (PTSD) are more likely to have psychiatric comorbidities and cardiovascular risk factors, including diabetes mellitus, hypertension, smoking and dyslipidemia.1 2 While stroke can trigger PTSD,3 prior to this study it was not known whether PTSD increases the risk for stroke. Methods of the study This was a longitudinal follow-up study to assess for development of stro...

Age Threshold for Increased Stroke Risk Among Patients With Atrial Fibrillation A Nationwide Cohort Study From Taiwan
This study hypothesized that the age threshold (65 years) used in the CHA2DS2-VASc system for initiating oral anticoagulants (OACs) might be lower in Taiwanese AF patients than in non-Asians.MethodsWe used the National Health Insurance Research Database in Taiwan to study 186,570 nonanticoagulated AF patients. There were 9,416 males with a CHA2DS2-VASc score of 0 and 6,390 females with a CHA2DS2-VASc score of 1. Their risk of ischemic stroke was analyzed with stratification on the basis of age.ResultsThe annual risks of ischemic stroke for males (score 0) and females (score 1) were 1.15% and 1.12%, respectively, and contin...
