Spontaneous calcific cerebral embolization revealing a calcified rheumatic mitral stenosis: a case report
ConclusionSpontaneous calcified cerebral emboli secondary to mitral valve leaflet calcifications is an extremely rare condition. Replacement of the valve is the only option to prevent recurrent emboli and outcomes are still to be determined.

Inflammation May Be the Culprit Behind Our Deadliest Diseases
In the early days of my medical residency, I met a man whom we’ll call Jason. He arrived to our emergency room on a holiday, nonchalant yet amiable, and complained of mild chest pain. Jason was tall and trim, with a strong South Boston accent and fingertips still faintly stained from his last home-improvement project. He was only 45 years old, but he looked much younger. He didn’t smoke, barely drank alcohol, and his cholesterol levels had always been normal. No one in his family had a history of heart disease. He asked us if we could work quickly—he wanted to be home for dinner with his daughters. [time-...

Transcatheter aortic valve replacement with or without anesthesiologist: results from a high-volume single center
Conclusion In patients at low or intermediate risk undergoing transfemoral TAVR, a safe procedure can be performed under local anesthesia without the presence of an anesthesiologist in the catheterization laboratory.

A 28-year-old male with spontaneous coronary artery dissection complicated by apical thrombus and acute ischemic stroke: An interesting case
Radiol Case Rep. 2022 Nov 2;18(1):246-249. doi: 10.1016/j.radcr.2022.10.001. eCollection 2023 Jan.ABSTRACTSpontaneous coronary artery dissection is described as the intramural bleeding that separates the layers of an epicardial coronary artery wall, either with or without an intimal tear. Atherosclerosis, iatrogenic damage, or trauma are not linked to this syndrome. Here we present a 28-year-old male with 1 month history stroke but no any chronic disease as well family history of heart disease who presented with 2 days' duration of typical cardiac chest pain. Based on an emergency electrocardiogram that showed biphasic T-w...

Adverse Events Associated with Cardiac Catheterization in Children Supported with Ventricular Assist Devices
Children on ventricular assist device (VAD) support can present several unique challenges, including small patient size, univentricular or biventricular congenital heart disease (1V- or 2V-CHD) and need for biventricular VAD (BiVAD) support. While cardiac catheterization can provide valuable information, it is an invasive procedure with inherent risks. We sought to evaluate the safety of catheterization in pediatric patients on VAD support. We performed a retrospective review of patients on VAD support who underwent catheterization at Lucile Packard Children’s Hospital between January 1, 2014 and September 1, 2019. Using...

First-pass perfusion cardiovascular magnetic resonance parameters as surrogate markers for left ventricular diastolic dysfunction: a validation against cardiac catheterization
ConclusionsPTTc may represent a non-invasive quantitative surrogate marker for the detection and assessment of diastolic dysfunction in LHD patients.Key Points• PTTc yielded the best diagnostic accuracy for diastolic dysfunction, with an optimal cutoff of 11.9 s, and a specificity of 100%.• PTTc and PTB were found to be independent predictors of LV diastolic dysfunction across different multivariate models with high reproducibility.• PTTc is a promising non-invasive surrogate marker for the detection and assessment of diastolic dysfunction in LHD patients.

Spontaneous Spinal Epidural Hematoma in an Adult Patient with Complex Congenital Heart Disease
We report the case of a 27-year-old man who developed sudden upper back pain with no specific precipitant. Five days later, he visited our emergency department complaining of weakness in both lower limbs and dysuria. He had a history of intracardiac repair and a Blalock-Park procedure for an interrupted aortic arch and ventricular septal defect in infancy. Additionally, he had undergone an aortic root dilatation and aortic valve replacement at the age of 10 because of progression of aortic and supra-aortic stenosis and had received chronic anticoagulation and antiplatelet therapy with warfarin and aspirin, respectively. An...

Impact of coronary artery disease on contractile function and ventricular ‐arterial coupling during exercise: Simultaneous assessment of left‐ventricular pressure–volume and coronary pressure and flow during cardiac catheterization
This study demonstrates that in the presence of flow-limiting CAD, exercise to maximal effort can lead to impairment of LV contractile function and a deterioration in VA coupling compared to a control cohort.
