Straight incision for extended descending and thoracoabdominal aortic replacement: novel and simple exposure with rib-cross thoracotomy
CONCLUSIONS This new exposure with straight incision with rib-cross thoracotomy provided excellent exposures for the long segment of the thoracoabdominal aorta, and it enabled extended replacement from the ascending aorta to the abdominal aorta.

Usual blood pressure, atrial fibrillation and vascular risk: evidence from 4.3 million adults
Conclusions:</strong> The association between elevated blood pressure and AF attenuates with increasing age. AF without antithrombotic usage is associated with an increased risk of eight vascular events.</span>

Aortic Arch Reconstructive Surgery with Conventional Techniques versus Frozen Elephant Trunk: A Systematic Review and Meta-Analysis
Conclusion Current evidence suggests that FET surgery is associated with lower mortality in patients with thoracic aneurysmal disease and dissections, without a significant increase in stroke, bleeding or operative times. However, the risk of spinal cord ischemia is increased in FET patients. A well powered randomized trial is needed to evaluate this evolving field. Teaser To our knowledge, we have performed the most detailed meta-analysis for the use of Frozen Elephant Trunk (FET) surgery, yielding a systematic review with the most number of patients studied. Our systematic review and meta-analysis uses the most up to dat...

Total arch replacement versus debranching thoracic endovascular aortic repair for aortic arch aneurysm: what indicates a high-risk patient for arch repair in octogenarians?
ConclusionsTAR in octogenarians with COPD and/or malignancy showed higher mortality rates; d-TEVAR is more appropriate in these situations. The prevention of perioperative stroke, which is related with poor prognosis in both the groups, is critical.

Aortic Arch Reconstructive Surgery With Conventional Techniques vs Frozen Elephant Trunk: A Systematic Review and Meta-Analysis
Conclusions Current evidence suggests that FET surgery is associated with lower mortality in patients with thoracic aneurysmal disease and dissections, without a significant increase in stroke, bleeding, or operative times. However, the risk of spinal cord ischemia is increased in patients who undergo FET. A well-powered randomized trial is needed to evaluate this evolving field.

Self-Reported Eczema in Relation with Mortality from Cardiovascular Disease in Japanese: the Japan Collaborative Cohort Study.
CONCLUSIONS: Self-reported frequent eczema was associated with increased risk of mortality from CHD, but not other major CVD, in a Japanese general population. Since steroid usage was not considered, future studies should include it as a potential confounding factor. PMID: 30700678 [PubMed - as supplied by publisher]

Outcome After Endovascular Repair of Ruptured Descending Thoracic Aortic Aneurysm: A National Multicentre Study.
CONCLUSIONS: This nationwide multicentre study of patients with rDTAA undergoing TEVAR showed acceptable short- but poor long-term survival. Adequate proximal and distal aortic sealing zones are important for technical success. High risk patients and post-operative complications need to be further addressed in an effort to improve outcome. PMID: 30910493 [PubMed - as supplied by publisher]

Probability of Uneventful Recovery after Elective Aortic Root Replacement for Aortic Aneurysm.
CONCLUSIONS: This study provides novel information regarding the probability of uneventful recovery while confirming low in-hospital mortality and stroke rates after elective aortic root replacement for aortic aneurysm. Importantly, uneventful recovery ensures excellent long-term survival. PMID: 32246934 [PubMed - as supplied by publisher]

Systolic, diastolic blood pressures predict risk of different cardiovascular diseases
Raised systolic and diastolic blood pressures may have different effects on different types of cardiovascular diseases and at different ages, according to new research involving 1.25 million patients from primary care practices. The new findings suggest that individuals with higher systolic blood pressures have a greater risk of intracerebral haemorrhage (stroke caused by bleeding within the brain tissue), subarachnoid haemorrhage (the deadliest form of stroke), and stable angina, whereas raised diastolic blood pressure is a better indicator of abdominal aortic aneurysm risk.

Cardiometabolic effects of genetic upregulation of the interleukin 1 receptor antagonist: a Mendelian randomisation analysis
Publication date: Available online 26 February 2015 Source:The Lancet Diabetes &amp; Endocrinology Background To investigate potential cardiovascular and other effects of long-term pharmacological interleukin 1 (IL-1) inhibition, we studied genetic variants that produce inhibition of IL-1, a master regulator of inflammation. Methods We created a genetic score combining the effects of alleles of two common variants (rs6743376 and rs1542176) that are located upstream of IL1RN, the gene encoding the IL-1 receptor antagonist (IL-1Ra; an endogenous inhibitor of both IL-1α and IL-1β); both alleles increase soluble IL-1Ra ...

Heterogeneous associations between smoking and a wide range of initial presentations of cardiovascular disease in 1 937 360 people in England: lifetime risks and implications for risk prediction
Conclusions The heterogeneous associations of smoking with different CVD presentations suggests different underlying mechanisms and have important implications for research, clinical screening and risk prediction.

The importance of and current trends in the endovascular program – A single center experience
Conclusion In the AAA group, the percentage of EVAR stabilized during the last 5 years at about 32% of the total number of treated patients. Given the excellent results of open carotid surgery and the unconvincing results of stenting trials, we consider open carotid surgery to be better than carotid artery primary stenting.
