Genetic knockout of myosin light chain kinase (MLCK210) prevents cerebral microhemorrhages and attenuates neuroinflammation in a mouse model of vascular cognitive impairment and dementia
We report here that genetic knockout (KO) of MLCK210 protects against cerebral microhemorrhages and neuroinflammation induced by chronic dietary hyperhomocysteinemia. Overall, the results are consistent with an accumulating body of evidence supporting MLCK210 as a potential therapeutic target for tissue barrier dysfunction and specifically implicate it in BBB dysfunction and neuroinflammation in a model of VCID.

Quantifying blood-brain barrier leakage in small vessel disease: Review and consensus recommendations
Publication date: Available online 25 April 2019Source: Alzheimer's & DementiaAuthor(s): Michael J. Thrippleton, Walter H. Backes, Steven Sourbron, Michael Ingrisch, Matthias J.P. van Osch, Martin Dichgans, Franz Fazekas, Stefan Ropele, Richard Frayne, Robert J. van Oostenbrugge, Eric E. Smith, Joanna M. WardlawAbstractCerebral small vessel disease (cSVD) comprises pathological processes of the small vessels in the brain that may manifest clinically as stroke, cognitive impairment, dementia, or gait disturbance. It is generally accepted that endothelial dysfunction, including blood-brain barrier (BBB) failure, is pivotal i...

The validity, reliability and clinical utility of a performance-based executive function assessment in people with mild to moderate dementia.
CONCLUSION: Our results indicated that the Chinese Multiple Errands Test is a reliable and valid instrument for assessing executive function in Chinese older people with dementia. PMID: 30990085 [PubMed - as supplied by publisher]

Dementia: Paradigm shifting into high gear
Publication date: Available online 9 April 2019Source: Alzheimer's & DementiaAuthor(s): Vladimir HachinskiAbstractRedressing the rising threat of dementia demands not only an increase, but a diversification of efforts. We need new approaches, trials, and partners. We cannot afford to continue to only round up the usual suspects, β amyloid, and tau and try to stop them with a single drug “silver bullet”. Dementia of late onset is not a disease, but an amalgam of interactive pathologies on the shifting background of aging, requiring multimodal targeting. Cerebrovascular diseases coexist and coact with all major neurodeg...

Change in Prescribing for  Secondary Prevention of Stroke and Coronary Heart Disease in Finnish Nursing Homes and Assisted Living Facilities
ConclusionsThe decline in cardiovascular medication use in residents with CHD and dementia suggests Finnish physicians are adopting a more conservative approach to the management of cardiovascular disease in the NH population.

Executive functions in the elderly with mild cognitive impairment: a systematic review on motor and cognitive inhibition, conflict control and cognitive flexibility.
CONCLUSIONS: The results of this review support the applicability of the four experimental tasks examined for the study of EFs in people with MCI. These paradigms are useful in research, diagnosis and therapeutic purposes, allowing obtaining an articulated EFs profile that can compromise the daily life in elderly. These EFs are not generally evaluated by standard assessment of MCI, but their evaluation can lead to a better knowledge of MCI and help in the diagnosis and treatment. Gender differences in presentation of behavioral and psychological symptoms in Alzheimer's disease in Taiwan.
CONCLUSIONS: The presentation of delusion and disinhibition in BPSD is associated with the female gender and staging of AD. Disinhibition was not necessarily associated with late stage of AD. PMID: 30912669 [PubMed - as supplied by publisher]

Factors associated with hospitalization for community-acquired pneumonia in home health care patients in Taiwan.
CONCLUSIONS: Our results revealed risk factors associated with hospitalization for CAP in HHC patients. In addition to chronic diseases, malnutrition is an important risk factor. Caregivers should make prompt assessments and take preventive measures for such patients. PMID: 30877643 [PubMed - as supplied by publisher]

2019 Alzheimer's disease facts and figures
This article describes the public health impact of Alzheimer's disease (AD), including incidence and prevalence, mortality and morbidity, use and costs of care, and the overall impact on caregivers and society. The Special Report examines the use of brief cognitive assessments by primary care physicians as a tool for improving early detection of dementia. An estimated 5.8 million Americans have Alzheimer's dementia. By mid-century, the number of people living with Alzheimer's dementia in the United States may grow to 13.8 million, fueled in large part by the aging baby boom generation. In 2017, official death certificates ...

Antidepressants for anxiety disorders in late-life: a systematic review
Anxiety disorders are encountered frequently in late life, and contribute to distress, disability, and increased mortality. Late-life GAD has been linked to increased risk of incident stroke [3]. Anxiety is associated with an increased risk for cognitive impairment and dementia in community dwelling older adults independent of psychosocial risk factors [6]. Despite these impairments, late-life anxiety disorders are largely untreated in primary care, with only 36.3% of recurrent cases in a community sample receiving treatment [7].

Atorvastatin in the treatment of lithium-induced nephrogenic diabetes insipidus: the protocol of a randomized controlled trial
Lithium is the gold-standard treatment for bipolar disorder, is highly effective in major depressive disorder, and is being investigated for therapeutic benefits in dementia, stroke, and even cancer. Approximately 350,000 Canadians use lithium and more could benefit. Despite this, clinicians are avoiding lithium, largely due to fear of renal toxicity, including irreversible chronic kidney disease (CKD). Nephrogenic Diabetes Insipidus (NDI) occurs in 20% of lithium users and independently predicts a 2-3 times increased risk of CKD.

Impact of a prudent lifestyle on the clinical outcomes of older men with bipolar disorder.
CONCLUSION: The excess morbidity and mortality of older men with BD seems to be mediated by non-prudent lifestyle practices. If future studies can demonstrate that this association is causal, then lifestyle interventions will need to become an integral part of the management of older adults with BD.
