Importance of Considering Competing Risks in Time-to-Event Analyses: Application to Stroke Risk in a Retrospective Cohort Study of Elderly Patients With Atrial Fibrillation Original Articles
Conclusions: The incidence of death without stroke was 9-fold higher than that of stroke, leading to biased estimates of stroke risk with traditional time-to-event methods. Statistical methods that appropriately account for competing risks should be used to mitigate this bias.

Does CHA2DS2-VASc score predict ischaemic stroke in chronic obstructive pulmonary disease?
Chronic obstructive pulmonary disease (COPD) is associated with a high risk of ischaemic stroke.1 CHA2DS2-VASc score is recommended and is widely used for stroke risk stratification in atrial fibrillation (AF) patients,2 but it has been shown to be an independent predictor of thrombo-embolic events in patients without AF, as well.3 In this issue ofEuropace, Huet al. performed a retrospective cohort study in a very big case sample (1492 COPD patients with AF and 50  343 COPD patients without AF), and reported that COPD patients with a higher CHA2DS2-VASc score were more likely to develop ischaemic stroke whether or not AF...

Chronic HCV infection is a risk factor of ischemic stroke
Conclusions: HCV infected patients are at higher and earlier risk of stroke. Inflammation is a key mediator. Clinicians in clinical practice and researchers in future trials should take into account these new findings.Highlights:

CHA2DS2-VASc score for ischaemic stroke risk stratification in patients with chronic obstructive pulmonary disease with and without atrial fibrillation: a nationwide cohort study
ConclusionsOur study is the first to show that the performance of CHA2DS2-VASc score in predicting ischaemic stroke is better for COPD patients without AF than for COPD patients with AF.

Asymptomatic Females Are at Higher Risk for Perioperative TIA/Stroke and Males Are at Higher Risk for Long-Term Mortality after Carotid Artery Stenting: A Vascular Quality Initiative Analysis
Int J Angiol DOI: 10.1055/s-0040-1712506The study aims to review the sex differences with respect to transient ischemic attack (TIA)/stroke and death in the perioperative period and on long-term follow-up among asymptomatic patients treated with carotid stenting (CAS) in the vascular quality initiative (VQI). All cases reported to VQI of asymptomatic CAS (ACAS) patients were reviewed. The primary end point was risk of TIA/stroke and death in the in-hospital perioperative period and in the long-term follow-up. The secondary end point was to evaluate predictors of in-hospital perioperative TIA/stroke and mortality on long-te...

A Novel Index in the Prediction of Pneumonia Following Acute Ischemic Stroke
CONCLUSIONS: This study presents factors that show a significant association with the occurrence of pneumonia in patients with acute ischemic stroke. The calculated PNEUMOINDEX consists of data obtained at admission, namely NYHA III and IV heart failure, COPD, generalized atherosclerosis, NIHHS score on admission, and CRP/Hgb ratio, and shows high prediction accuracy in predicting hospital-acquired pneumonia in ischemic stroke patients.PMID:36430028 | DOI:10.3390/ijerph192215306

Differential association of chronic obstructive pulmonary disease with myocardial infarction and ischemic stroke in a nation-wide cohort
Chronic obstructive pulmonary disease (COPD) has been associated with an increased risk of cardiovascular disease, which has been suggested to relate to both similar risk factors and to similar pathophysiological mechanisms . The latter notion has also received support from pharmacoepidemiological studies, suggesting reduced cardiovascular risk by respiratory medications . However, the full impact of COPD on myocardial infarction (MI) and stroke in a population-based cohort remains to be fully investigated. The aim of the present study was to outline the association of COPD with MI and stroke. To this end, we analyzed the ...

Cause of Death and Predictors of All-Cause Mortality in Anticoagulated Patients With Nonvalvular Atrial Fibrillation: Data From ROCKET AF Arrhythmia and Electrophysiology
Conclusions In a large population of patients anticoagulated for nonvalvular atrial fibrillation, 7 in 10 deaths were cardiovascular, whereas <1 in 10 deaths were caused by nonhemorrhagic stroke or systemic embolism. Optimal prevention and treatment of heart failure, renal impairment, chronic obstructive pulmonary disease, and diabetes may improve survival. New-Onset Atrial Fibrillation Is a Risk Factor of Ischemic Stroke in Chronic Obstructive Pulmonary Disease
In conclusion, COPD patients suffering from new-onset AF had an increased incidence of IS in the population observation study. New-onset AF was an omit risk factor for IS in COPD in the Chinese population.PMID:35206997 | DOI:10.3390/healthcare10020381

Abstract 226: Strokes Worse in Women at 24 hours but Severity Reduced in Younger Women With Hormone Therapy Session Title: Poster Session II
Conclusion: Women in this study had more severe 24 hour AIS outcomes than men but this improved with HRT use in women younger than 80 years. This cannot be fully explained by age differences as there could be other underlying factors. Improving 24 hour NIHSS may correctly predict later outcomes following AIS. Further study of HRT use in AIS outcomes is justified.

Prevalence of atrial fibrillation and the HATCH score : Intensified monitoring of patients with high HATCH score.
CONCLUSION: The HATCH score correlates with the occurrence of AF, since the prevalence of AF rises with rising score values. Therefore, the HATCH score may be used to select patients for intensified ECG monitoring. Moreover, the score may also be used for stroke risk assessment, as none of the patients with a low HATCH score suffered a stroke. PMID: 25939435 [PubMed - as supplied by publisher]

Sex-Specific Outcomes of Transcatheter Aortic Valve Replacement With the SAPIEN 3 Valve: Insights From the PARTNER II S3 High-Risk and Intermediate-Risk Cohorts
Conclusions The study found no apparent sex-specific differences in survival or stroke in this trial of TAVR. This may reflect the changing demographic of patients enrolled, use of newer-generation valves with more sizes available, and more accurate valve sizing techniques.
