Cerebral ischemic events in patients with pancreatic cancer: A retrospective cohort study of 17 patients and a literature review
Abstract: Stroke is a dramatic complication of pancreatic cancer with mechanisms related to oncological disease. A better description of the characteristics of cerebrovascular events would help better understand the pathogeny and protect vulnerable patients. We thus conducted a descriptive analysis of clinical, biological, and radiological features of patients from our centers and literature. We reviewed consecutive cases of patients who presented cerebrovascular events and pancreatic cancer in 4 stroke units in Lorrain (France) between January 1, 2009 and March 31, 2015, and all reported cases of literature. We identifie...

Endovascular management of cerebral septic embolism: Three recent cases and review of the literature. (P3.299)
Conclusions:The literature reports overall favorable outcomes and recanalization rates with endovascular management of cerebral septic emboli causing acute ischemic stroke, but one should remain cautious because these outcomes may be influenced by publication bias. Mechanical embolectomy could be considered in select cases even in the setting of prior infarcts and/or systemic anticoagulation, upon careful informed consent with the patient and/or family/health care proxy. Mechanical embolectomy is likely to be more effective than intravenous or intra-arterial thrombolysis for achieving recanalization because of the expected...

Early vs Late Surgery for Patients With Endocarditis and Neurological Injury: A Systematic Review and Meta-analysis
ConclusionsAvailable observational data support delaying surgery by 7-14 days if possible in IE complicated by ischemic stroke and> 21 days in hemorrhagic stroke to decrease perioperative mortality and neurological exacerbation rates.

284 * the impact of preoperative neurological events in patients suffering from native infective valve endocarditis
Conclusions: NVE complicated by neurological events remains a challenging disease with high mortality and morbidity. Cardiac surgical intervention is still associated with impaired outcome but seems not to aggravate preoperative existing neurological symptoms in the observed time interval.

1990-2010 Global Cardiovascular Disease Atlas
Worldwide, the cardiovascular diseases (CVDs) contributing most to the total global burden of disease in 2010 were ischemic heart disease (5.2% of all disability-adjusted life years [DALYs] lost) and stroke (4.1% of all DALYs). The other major CVDs were hypertensive heart disease, cardiomyopathies, rheumatic heart disease, atrial fibrillation, aortic aneurysm, peripheral vascular disease, and endocarditis. The highest per capita CVD burden fell upon the Eastern Europe and Central Asia regions (). In the large populations of the South Asia and North Africa and Middle East regions, the absolute burden of CVDs is high and mor...

Long-Term Clinical Outcome of Major Adverse Cardiac Events in Survivors of Infectious Endocarditis: A Nationwide Population-Based Study.
CONCLUSIONS: -Despite treatment, the risk of long-term major adverse cardiac events was substantially increased in IE survivors. PMID: 25223982 [PubMed - as supplied by publisher]

300 * influence of different pacemaker stimulation modes in patients with ventricular hypertrophy
Conclusion: An increase in the heart rate does not lead to a notable drop in stroke volume postoperatively in left-ventricular hypertrophy; hence a rise in cardiac output can be anticipated up to a rate of 100 bpm. A standardised response in terms of an ideal pacemaker stimulation mode could not be identified.

Neurological complications of infective endocarditis in Burkina Faso. Clinical features, management and evolutionary profile.
CONCLUSION: This study shows that neurological complications during infective endocarditis are frequent, dominated by stroke with a high mortality. PMID: 25702236 [PubMed - as supplied by publisher]

Clinical Outcomes in 3343 Children and Adults with Rheumatic Heart Disease from 14 Low and Middle Income Countries: 2-Year Follow-up of the Global Rheumatic Heart Disease Registry (the REMEDY study).
CONCLUSIONS: -Patients with clinical RHD have high mortality and morbidity despite being young; those from low and lower-middle income countries had a poorer prognosis associated with advanced disease and low education. Programs focused on early detection and treatment of clinical RHD are required to improve outcomes. PMID: 27702773 [PubMed - as supplied by publisher]

Neuroimaging Characteristics of Intracranial Hemorrhages and Microhemorrhages in Infective Endocarditis (P2.274)
Conclusions:Intracranial macro- or micro-hemorrhages are seen in 73 (63%) persons with IE. The mortality of intracranial hemorrhage is high but the perioperative stroke risk appears low.Disclosure: Dr. Cho has nothing to disclose. Dr. Marquardt has nothing to disclose. “A contemporary description of staphylococcus aureus prosthetic valve endocarditis. Differences according to the time elapsed from surgery”
Staphylococcus aureus prosthetic valve endocarditis (SAPVE) has a poor prognosis. There are no large series that accurately describe this entity. This is a retrospective observational study on a prospective cohort from 3 Spanish reference hospitals for cardiac surgery, including 78 definitive episodes of left SAPVE between 1996 and 2016. Fifty percent had a Charlson Index score>5; 53% were health care-related. Twenty percent did not present fever. Complications at diagnosis included: severe heart failure (HF, 29%), septic shock (SS, 17.9%), central nervous system abnormalities (19%), septic metastasis (4%). Hemorrhagic s...

Transcatheter aortic valve implantation versus surgical aortic valve replacement in patients with severe aortic stenosis: a systematic review and meta-analysis
Conclusions No overall change in the risk of death from any cause or cardiovascular mortality was identified but 95% CIs were often wide, indicating uncertainty. TAVI may reduce the risk of certain side effects while SAVR may reduce the risk of others. Most long-term (5-year) results are limited to older patients at high surgical risk (ie, early trials), therefore more data are required for low risk populations. Ultimately, neither surgical technique was considered dominant, and these results suggest that every patient with SAS should be individually engaged in SDM to make evidence-based, personalised decisions around thei...

Etiology, clinical characteristics, and outcome of infective endocarditis: 10-year experience from a tertiary care center in Pakistan
In conclusion, our study provided valuable data on the clinical characteristics and outcomes of patients with IE in a developing country. S. aureus was the most common causative agent. Heart failure and stroke were the most common complications. The presence of prosthetic valves, history of chronic kidney disease, ischemic heart disease and previous stroke were associated with a significantly increased risk of mortality. Surgical management was not associated with improved outcomes.PMID:35347974 | DOI:10.4081/monaldi.2022.2212

E-136 Morphology of mycotic aneurysms and outcomes
ConclusionPatients with secured mycotic aneurysms are less likely to have in-hospital mortality from postoperative hemorrhage and more likely to have better outcomes upon discharge. This snapshot study warrants further investigation to determine the optimal safe timing between securing IIAs to valvular surgery.Disclosures N. Krothapalli: None. M. Tsikvadze: None. N. Akram: None.
