CONCLUSIONS: Cardiac involvement in m.3243A>G carriers has been only rarely systematically studied, which is perhaps why the incidence of cardiac diseases in MELAS is lower than would be expected. Myocardial abnormalities are much more frequent than arrhythmias or conduction defects. All symptomatic and asymptomatic m.3243A>G carriers should be systematically investigated for cardiac disease. The contribution of systematic implementation of cardiovascular explorations for the detection of cardiac sources of embolism in patients with stroke: About 230  patients.
CONCLUSION: Cardiovascular explorations remain indispensable in the workup of all ischemic strokes even if their therapeutic effect is modest. Super High Dose Beta-blocker Administration for the Patients with Dilated Cardiomyopathy
First case is a 25-year-old female with dilated cardiomyopathy. Left ventricular end-diastolic diameter (LVEDD) was 74  mm and ejection fraction (EF) was 29%. Carvedilol was already uptitrated to 20 mg twice daily. Right heart catheterization (RHC) revealed very low cardiac output (C.O. 2.82 l/min), cardiac index (C.I. 1.47 l/min/m2), stroke volume (SV 29 ml) and rapid heart rate (HR 96/min) despite maximu m dosage of carvedilol in Japan. Carvedilol was uptitrated to 50 mg twice daily and additional 10 mg bisoprolol twice daily was started under administration of low-dose dobutamine.

Thirty years of heart transplantation at the University Medical Centre Utrecht.
CONCLUSION: Over the past 30 years, patient and donor demographics and underlying diseases have shifted substantially. Furthermore, the increase in waiting time due to lack of available donor hearts has led to a rise in the use of LVADs as bridge to transplant. Importantly, an improvement in survival rates is found over time which could be explained by better immunosuppressive therapy and improvements in follow-up care. The Effects of Levosimendan and Sodium Nitroprusside Combination on Left Ventricular Functions After Surgical Ventricular Reconstruction in Coronary Artery Bypass Grafting Patients.
CONCLUSION: This study showed that LS and SNP combination impressive increase in left ventricular systolic and diastolic functions including LVEF. Impact of papillary muscles on ventricular function measurements in 3T cardiac magnetic resonance
Conclusion Global CMR LV parameters are significantly affected by whether TPM are considered as part of the LV blood pool or as part of LV mass. Therefore, a consistent method of LV cavity delineation may be crucial during longitudinal follow-up to avoid misinterpretation and erroneous clinical decision-making.

Patients Selection for Left Ventriculoplasty and Mitral Valve Surgery in Non-Ischemic Dilated Cardiomyopathy
The operative indication for dilated cardiomyopathy (DCM) is controversial. Subjects were 20 non-ischemic DCM patients. Left ventriculoplasty and mitral valve repair were conducted. Cardiac index derived from forward stroke volume significantly increased after surgery (from 1.8  ± 0.6 to 2.4 ± 0.6, P = .001). One-year survival rate was 58%. Slope in preload recruitable stroke work relationship (Mw) was an independent predictor for survival after surgery (HR, 0.936; 95%CI, 0.894–0.980; P = .005). Cut-off value of Mw for one-year survival was 42.
