Conclusion Our results demonstrate the need for service development to facilitate adherence to RCOphth guidance. Up to 40% of patients started on HCQ have baseline risks for retinopathy, most due to dosing>5mg/kg/day warranting yearly screening. Baseline retinal testing is no longer recommended for hydroxychloroquine users in the United Kingdom
The revised BSR DMARD guidelines introduced substantial changes to monitoring recommendations for hydroxychloroquine [1]. The guidelines, which reflected the Royal College of Ophthalmologists (RCOphth) 2018 recommendations on retinal monitoring [2], introduced a significant burden on clinical teams, and some services were unable to deliver them. The RCOphth has now published updated guidelines on retinal monitoring for hydroxychloroquine and chloroquine users in the United Kingdom (December 2020) [3]. Guidelines on prescribing and monitoring antimalarials in rheumatic diseases: a systematic review.
CONCLUSIONS: The newest recommendations are heterogeneous suggesting lower hydroxychloroquine dosing. Retinal toxicity is irreversible and the risk increases over time. Annual screening after five years with automated VF and SD-OCT may be warranted to detect early changes and discontinue therapy if necessary. Pachymeter Use and Disinfection Practice in the United Kingdom: A National Survey
Conclusions: There is a large variation in methods and duration of tip disinfection with only a few units following the Royal College of Ophthalmologists (RCOphth) guidelines on pachymeter disinfection. The majority of eye units use alcohol/chlorine-based wipes for cleaning the pachymetry heads which is against the current recommended guidelines. COVID-19: Ophthalmological Aspects of the SARS-CoV 2 Global Pandemic.
CONCLUSION: Within a short period, a growing body of articles has started to elucidate the ophthalmological implications of COVID-19. As the eye can represent a route of infection (actively via tears and passively via the nasoacrimal duct), ophthalmological care has to undergo substantial modifications during this pandemic. Retinopathy of prematurity occurrence and evaluation of screening policy in a large tertiary Greek cohort
