Species distribution and antifungal susceptibility profiles of yeasts isolated from onychomycosis: a cross-sectional study with insights into emerging species
Antonie Van Leeuwenhoek. 2023 Dec 28;117(1):6. doi: 10.1007/s10482-023-01914-0.ABSTRACTCandida onychomycosis is a common fungal infection affecting the nails, primarily caused by Candida (C.) species. Regarding the increasing trend of Candida onychomycosis and the antifungal resistant phenomenon in recent years, this study aims to evaluate the epidemiological characteristics of Candida onychomycosis, the distribution of emerging species, and the antifungal susceptibility profiles of isolates. Onychomycosis caused by yeast species was confirmed through direct examination and culture of nail scraping among all individuals su...

Systematic review and QSPR analysis of chemical penetration through the nail to inform onychomycosis candidate selection
Drug Discov Today. 2023 Nov 22:103844. doi: 10.1016/j.drudis.2023.103844. Online ahead of print.ABSTRACTRecalcitrant nail plate infections can be life-long problems because localizing antifungal agents into infected tissues is problematic. In this systematic review, guided by the SPIDER method, we extracted chemical nail permeation data for 38 compounds from 16 articles, and analyzed the data using quantitative structure-property relationships (QSPRs). Our analysis demonstrated that low-molecular weight was essential for effective nail penetration, with <120 g/mol being preferred. Interestingly, chemical polarity had li...

Chest. 2023 Nov;164(5):e135-e138. doi: 10.1016/j.chest.2023.06.020. Epub 2023 Nov 7.ABSTRACTA 28-year-old woman with a history of congenital hip dysplasia was referred for evaluation of recurrent bronchitis. She had undergone left hip replacement with titanium implants 11 years prior to presentation. The patient reported frequent bouts of bronchitis, sinusitis, and left-sided nontender facial swelling that started after the hip replacement surgery. She also reported nail discoloration of her left first toenail 1 year after this procedure, and nail discoloration of her right first toenail 3 years after the procedure. She wa...

The safety of oral antifungals for the treatment of onychomycosis
This article identifies possible hepatic, renal, cutaneous, cardiovascular, neurological, hemopoietic and obstetric adverse events. We have also compared the rates of hepatotoxicity, clinically apparent liver injury, and alanine transaminase elevations between oral antifungals, and recommendations for hepatic monitoring.EXPERT OPINION: We recommend laboratory testing of liver function tests prior to the administration of any oral antifungals, especially when clinically indicated. In the event of a first treatment failure, the diagnosis of onychomycosis must be confirmed, and consideration given to antifungal susceptibility...

Trichophyton: Changing nomenclature and practical implications
Manas Chatterjee, Debatri DattaIndian Journal of Dermatology 2023 68(5):503-507 Trichophyton interdigitale had been regarded as anthropophilic, mainly causing non-inflammatory tinea unguium and tinea pedis. T. mentagrophytes, thought to be zoophilic, were regarded as responsible for more inflammatory dermatophytosis. Indian terbinafine-resistant strains, identified with ribosomal internal transcribed spacer as 'genotype VIII', have recently been termed Trichophyton indotineae based on clinical and mycological features. Some of these have shown selective azole resistance as well. Phenotypic studies ha...

Cutaneous dysbiosis and dermatophytosis: The unexplored link
Aparajita Ghosh, Saumya PandaIndian Journal of Dermatology 2023 68(5):508-514 The skin, besides being the largest interface between the body and the external environment, also forms an ecological niche which is populated by almost a trillion microorganisms. These, collectively known as the cutaneous microbiome, form a dynamic yet well-controlled system that resists invasion by pathogenic microorganisms, functioning as the so-called ‘microbiological barrier’, modulating the body’s immune response, indirectly playing a crucial role in the pathogenesis of several inflammatory diseases. ...

Utility of devices for onychomycosis: a review
J Dermatolog Treat. 2023 Dec;34(1):2265658. doi: 10.1080/09546634.2023.2265658. Epub 2023 Oct 8.ABSTRACTOnychomycosis is difficult to treat due to long treatment durations, poor efficacy rates of treatments, high relapse rates, and safety issues when using systemic antifungal agents. Device-based treatments are targeted to specific regions of the nail, have favorable safely profiles, and do not interfere with systemic agents. They may be an effective alternative therapy for onychomycosis especially with increasing reports of squalene epoxidase gene mutations and potential resistance to terbinafine therapy. In this review, ...
