Dermatology urgent care model reduces costs and healthcare utilization for psychodermatology patients - a retrospective chart review
Background: Patients with psychiatric dermatoses may be high users of healthcare, especially emergency services. A dermatology urgent care model may reduce healthcare utilization in this population. Objective: To determine whether a dermatology urgent care model can reduce healthcare utilization among patients with psychiatric dermatoses. Methods: We conducted a retrospective chart review of patients seen in dermatology urgent care at Oregon Health and Science University between 2018 and 2020 with diagnoses of Morgellons disease and neurotic excoriations. Rates of diagnosis-related healthcare visits and emergency departmen...

New Insights into Delusional Infestation
J Dermatolog Treat. 2022 Aug 19:1-4. doi: 10.1080/09546634.2022.2115835. Online ahead of print.ABSTRACTThe recently proposed Koo-Brownstone staging system for Delusional Infestation (DI) is a large step forward in Dermatology. It proposes that patients' presentations can be separated into various categories along a spectrum, and that each of these categories are approached differently. This system explains longstanding variation in the reported response to treatment of patients who present with concerns of infestation. To develop a consensus across the medical community, the authors propose that the terms are discussed in ...

The challenge of Morgellons disease: A patient with clinicopathologic correlation
Morgellons disease is a rare condition characterized by patient-reported multicolored fibers and other nonorganic particles or organic particles embedded in and protruding from diffuse skin ulcerations. Although the scientific community is prone to believe that Morgellons disease is a psychiatric disorder, an infectious pathogenesis associated with Borrelia burgdorferi in the setting of Lyme disease has also been proposed. The histopathology is usually considered as nonspecific. To illustrate this condition, we present the case of an adult woman with significant ulcerative skin lesions and cicatricial changes on the face, ...

Treatment of Morgellons disease with doxycycline
We report a middle-aged Caucasian female who developed symptoms of MD in the days following exposure to a tick bite after spending an afternoon hiking through a wooded area. She was subsequently treated with a course of Doxycycline and found on two-week follow-up to have complete remission of her symptoms. This case report further supports the theory for an infectious etiology of MD and encourages future studies into its pathophysiology. (Source: Clinical Case Reports)
