Fatal iatrogenous neonatal massive air embolism with diffuse emphysema, pneumothorax, pneumomediastinum and pneumoperitoneum
ConclusionFor a forensic-clinical discussion, I consider this rare presentation of a massive lethal air plurifocal embolism to be a necessary contribution to the understanding of professional, educational and organizational communication failure. Alveolar overdistension with hyperinflation of oxygen led to imposibility of spontaneous and assisted ventilation, massive air embolism and consequent complications that led to death without response to resuscitation measures. An operational maintenance approach for improving physiological monitor by HFMEA process: an empirical case study
CONCLUSIONS: This study concludes a systematic reference in malpractices caused by maintenance negligence. Establishing a regional registry for neonatal encephalopathy: impact on identification of gaps in practice
CONCLUSIONS: The registry has allowed for the identification of areas of variation in clinical practices, which have guided ongoing quality improvement projects. The authors advocate for the establishment of local and regional registries to standardize and improve NE patient care.
