Infectious Complications in Lung Transplant Recipients
Infection remains a common cause of death throughout the lifespan of a lung transplant recipient. The increased susceptibility of lung transplant recipients is multifactorial including exposure of the graft to the external environment, impaired mucociliary clearance, and high levels of immunosuppression. Long-term outcomes in lung transplant recipients remain poor compared with other solid organ transplants largely due to deaths from infections and chronic allograft dysfunction. Antibacterial, antifungal, and antiviral prophylaxis may be used after lung transplantation to target a number of different opportunistic infectio...

Outpatient Pharmacologic Management of Lung Transplant Candidates on the Waiting List
The medical care of patients awaiting lung transplantation is complex and requires the treatment of active medical conditions, including lung disease, while at the same time maintaining candidacy for transplantation. Some medications that would otherwise be considered routine may create undesirable challenges or complications in the perioperative setting. Therefore, a comprehensive assessment of the risks and benefits of these medications must take into account both their potential utility in managing a patient ’s current disease state, as well as the risks of compromising postlung transplant outcomes. In this review, we...

Nonallograft Complications of Lung Transplantation
This article focuses on common nonallograft complications with the highest impact on short- and long-term outcomes after transplantation. These include diabetes mellitus, hypertension, dyslipidemia, kidney disease (acute and chronic), and malignancy. We discuss evidence-based strategies for the prevention, diagnosis, and management of these nonallograft complications in this article. (Source: Thoracic Surgery Clinics)

Lung Transplantation: Advances Through Collaboration
It “takes a village” to care for a lung transplant recipient. With advances in surgical technique for implantation, donor organ preservation and recovery, immunosuppression, and medical management over the past six decades, there has been a remarkable improvement in outcomes following lung transpla ntation. Today, the overall median survival following lung transplantation is 6.7 years. In fact, at some centers, the median survival has approached 10 years with a handful of lung recipients surviving close to 30 years posttransplant. (Source: Thoracic Surgery Clinics)

Inpatient Management of the Acutely Decompensating Lung Transplant Candidate
Lung allocation in the US changed nearly 15  years ago from time accrued on the waiting list to disease severity and likelihood of posttransplant survival, represented by the lung allocation score (LAS). Notably, the risk of death within a year plays a stronger role on the score calculation than posttransplant survival. While this change was associated with the intended decrease in waitlist mortality (most recently reported at 14.6%), it was predictable that transplant teams would have to care for increasingly older and complex candidates and recipients. This urgency-based allocation also led centers to routinely consider...

Lung Transplantation: Moving Forward yet Still Impeded by Infection and Rejection
We are excited to bring you the “Lung Transplantation: Advances Through Collaboration” issue for the Thoracic Surgery Clinics. Invited guest editors, UCSF Thoracic Surgeon Jasleen Kukreja, MD, MPH, and Transplant Pulmonologist Aida Venado, MD, MAS, have brought together a panoply of current topics about lung transplantation. T hrough invited experts in the field, our editors provide a state-of-the-union summary for surgical management of advanced lung disease, including chronic problems like pulmonary fibrosis and COPD, and acute indications, like acute respiratory distress syndrome, including, selectively, as a compli...

Lung transplantation for acute respiratory distress syndrome
In this review, we discuss the outcomes of patients with severe acute respiratory distress syndrome. We discuss evidence which suggests that a significant proportion of patients with ARDS develop end-stage lung disease and die of pulmonary complications. In carefully selected patients with permanent lung damage, lung transplant can be a life-saving treatment. (Source: Thoracic Surgery Clinics)
