Barriers to Lung Cancer Screening Access from the Perspective of the Patient and Current Interventions
In the context of the Conceptual Model for Lung Cancer Screening Participation, this article describes patient barriers to lung cancer screening highlighting current interventions. Patient barriers include cognitive factors (lack of awareness, limited information/misinformation, and low perceived risk), factors related to access (logistical issues, no provider recommendation, cost, and other financial/social factors), and psychological factors (fear, fatalism, lung cancer worry, and stigma). Current interventions include the use of educational materials/presentations to address cognitive barriers, use of direct outreach an...

Liquid Biopsy as an Adjunct to Lung Screening Imaging
Current lung cancer screening protocols use low-dose computed tomography scans in selected high-risk individuals. Unfortunately, utilization is low, and the rate of false-positive screens is high. Peripheral biomarkers carry meaningful promise in diagnosing and monitoring cancer with added potential advantages reducing invasive procedures and improving turnaround time. Herein, the use of such blood-based assays is considered as an adjunct to further utilization and accuracy of lung cancer screening. (Source: Thoracic Surgery Clinics)

Artificial Intelligence and Machine Learning in Lung Cancer Screening
This article reviews the status and future directions of AI/ML tools in the lung cancer screening workflow, focusing on determining screening eligibility, radiation dose reduction and image denoising for low-dose chest computed tomography (CT), lung nodule detection, lung nodule classification, and determining optimal screening intervals. AI/ML tools can assess for chronic diseases on CT, which creates opportunities to improve population health through opportunistic screening. (Source: Thoracic Surgery Clinics)

Clinical Adjuncts to Lung Cancer Screening
The updated US Preventive Services Task Force guidelines on lung cancer screening have significantly expanded the population of screening eligible adults, among whom the balance of benefits and harms associated with lung cancer screening vary considerably. Clinical adjuncts are additional information and tools that can guide decision-making to optimally screen individuals who are most likely to benefit. Proposed adjuncts include integration of clinical history, risk prediction models, shared-decision-making tools, and biomarker tests at key steps in the screening process. Although evidence regarding their clinical utility ...

Access to Lung Cancer Screening
Rural and racial/ethnic minority communities experience higher risk and mortality from lung cancer. Lung cancer screening with low-dose computed tomography reduces mortality. However, disparities persist in the uptake of lung cancer screening, especially in marginalized communities. Barriers to lung cancer screening are multilevel and include patient, provider, and system-level barriers. This discussion highlights the key barriers faced by rural and racial/ethnic minority communities. (Source: Thoracic Surgery Clinics)

Tracheobronchoplasty
Tracheobronchomalacia (TBM) is an increasingly recognized abnormality of the central airways in patients with respiratory symptoms. Severe TBM in symptomatic patients warrants screening dynamic CT of the chest and/or awake dynamic bronchoscopy. The goal of surgical repair is to restore the C-shaped configuration of the airway lumen and splint or secure the lax posterior membrane to the mesh to ameliorate symptoms. Robotic tracheobronchoplasty is safe and associated with improvements in pulmonary function and subjective improvement in quality of life. (Source: Thoracic Surgery Clinics)

Gastroesophageal Reflux Disease in 2023
Gastroesophageal reflux disease (GERD) is among the most prevalent diseases in the United States. Mainstay therapy is lifestyle modification and medical therapy. If patients have GERD despite medical therapy, appropriate testing should be performed to determine if surgical or endoscopic therapy will provide improvement in their symptoms. Gold standard therapy is a minimally invasive fundoplication. Patients with body mass index (Source: Thoracic Surgery Clinics)

Thoracic Surgery: Where ’s the Controversy?
What ’s controversial in our field? Aren’t we all on the same page when it comes to patient care? No, we’re not; thus, Drs Lana Schumacher and David Griffin have offered up 12 areas where management may differ between thoracic surgeons or between surgeons and their medical specialty colleagues. The evidence doesn’t always support one consensus, and (apparently) there can be more than one way to skin a cat, so they say. With relevance to us, however, in the operating room and clinic is that there may be several ways to approach esophageal, lung, airway, and mediastinal disease, but robust ev idence lends guidance fo...

The More Things Change …
It is our great pleasure to introduce the latest issue of Thoracic Surgery Clinics entitled “Controversies in Thoracic Surgery.” Surgical technique continues to evolve, and increased adoption of minimally invasive techniques drive improvements in surgical care. Furthermore, exciting advances in collaborative fields, such as medical oncology and gastrointestinal endoscopy, have dramatic ally impacted how we practice as thoracic surgeons and have, in some cases, kept us (and our patients) out of the operating room.
