Becoming a Cardiothoracic Surgical Educator
Cardiothoracic training continues to evolve and is a completely different teaching environment than prior. Cardiothoracic surgical educators are tasked to ensure that all levels of learners are appropriately trained. To be an effective surgical educator, one must expand their skills beyond being content experts but also acquire knowledge of educational theory and formal training in how to teach effectively. Furthermore, applying a scholarly approach and engaging in scholarship differentiate a cardiothoracic surgical educator from a teacher. Therefore, the surgical educator academic track is different from a surgical scient...

Integrating Advocacy into Your Practice
In the last 60  years, health care has evolved due to many trends including the introduction of third-party payers, a progressively aging society, advancing technology, emerging recognition of social issues of race/gender/poverty, the relative decline in the physician workforce, rising health care costs, ongoing consolidation of health care entities, and the corporatization of health care delivery. This has led to problems in health care delivery with respect to cost, quality, and access. Many medical leaders feel it is now the duty of the physician to go beyond the classic patient-doctor clinical responsib ility and work...

Diversity in the Cardiothoracic Surgery Workforce
Within the cardiothoracic surgery workforce, there are significant gaps in the numbers of women and underrepresented in medicine minorities, but some progress has been made in gender diversity at the resident level. Individual surgeons play an important role in combatting discrimination and harassment, while also promoting women and minorities through mentorship and sponsorship. More importantly, a multifaceted and structured approach is needed to increase diversity at the institutional level with strategies to create a culture of inclusion, working to retain underrepresented minority and female surgeons, and eliminating b...

What Is the “Right” First Job?
It is a daunting task to find the “right” first job. However, the foundation of the search is similar to that of the interview and match process for residency and fellowship. Does the job opportunity have the makeup of clinical and research opportunities, case mix, support and culture that will set the trainee up to fulfill his or her early career goals? Does the position seem like a good fit? The variation occurs with (1) the mystery behind it—there are scarce resources available on the topic, and (2) the logistics: where and when to look; the interview process. The Exit Strategy
Retiring from any occupation is difficult, especially one that you love. The majority of cardiothoracic surgeons love what they do every day. It has been said that if you choose a job you love, you never have to work another day in your life. Once a date is determined, preparation, particularly financial, is paramount for a successful outcome. Thoughtful decisions need to be made regarding health benefits and retirement plans [401(k)/403(b)]. Transitions to retirement programs have been instituted in several schools of medicine. Establishing an academy for retired faculty can be an enriching experience for the members and ...

Finding the Right Mentor
Quality mentorship is essential for a successful career in cardiothoracic surgery. From the premedical phase to the position of senior faculty, there are many benefits to having mentors who can provide insight, promote career advancement and facilitate professional opportunities. It is important to distinguish between a mentor and sponsor in seeking this career guidance because both are beneficial but serve different purposes. By being clear about one ’s professional goals, the mentor–mentee relationship can be optimized and lead to a fulfilling and productive career. (Source: Thoracic Surgery Clinics)

Uniportal Robotic Lung Resection Techniques
Uniportal video-assisted thoracic surgical (U-VATS) and telerobotic techniques have become widely adopted strategies for lung resection and represent a natural progression born of advancing technologic innovation and decades of expanding clinical experience. Combining the best that each approach offers may be the next logical step in the evolution of minimally invasive thoracic surgery. Two parallel efforts are underway: one that combines the traditional U-VATS incision with a multi-arm telerobotic platform and one that utilizes a new single-arm device. Feasibility and refinement of surgical technique will need to be achie...

Single Setting Robotic Lung Nodule Diagnosis and Resection
Lung cancer remains the leading cause of cancer-related deaths. Early tissue diagnosis followed by timely therapeutic procedures can have a significant impact on overall survival. While robotic-assisted lung resection is an established therapeutic procedure, robotic-assisted bronchoscopy is a more recent diagnostic procedure that improves reach, stability, and precision in the field of bronchoscopic lung nodule biopsy. The ability to combine lung cancer diagnostics with therapeutic surgical resection into a single-setting anesthesia procedure has the potential to decrease costs, improve patient experiences, and most import...

Lung Xenotransplantation
Although efforts have been made to expand the pool of donor lung allografts for human lung transplantation, a shortage remains. Lung xenotransplantation has been proposed as an alternative approach, but lung xenotransplantation in humans has not yet been reported. In addition, significant biological and ethical barriers will have to be addressed before clinical trials can be undertaken. However, significant progress has been made toward addressing biological incompatibilities that present a barrier, and recent advances in genetic engineering tools promise to accelerate further progress. Three-Dimensional Printing Applications in Thoracic Surgery
Advances in technology allowing the combination of medical imaging and three-dimensional printing have greatly benefitted thoracic surgery, allowing for the creation of complex prostheses. Surgical education is also a significant application of three-dimensional printing, especially for the development of simulation-based training models. Aiming to show how three-dimensional printing can benefit patients and clinicians in thoracic surgery, an optimized method to create patient-specific chest wall prosthesis using three-dimensional printing was developed and clinically validated. An artificial chest simulator for surgical t...

Bronchoscopic Lung Volume Reduction
Bronchoscopic lung volume reduction (BLVR) for the treatment of emphysema was originally developed in the early 2000s as a minimally invasive alternative to lung volume reduction surgery. Endobronchial valves for BLVR are an advancing “guideline treatment” in the treatment of advanced emphysema. Placement of small, one-way valves into segmental or subsegmental airways can induce lobar atelectasis for portions of diseased lung. This results in the reduction of hyperinflation along with improvements in diaphragmatic curvature a nd excursion. (Source: Thoracic Surgery Clinics)

Emerging Technologies in Thoracic Surgery
The evolution of our practice as thoracic surgeons continues to lean toward more minimally invasive and endoscopic procedures. During the last decade, we have witnessed a transition from video-assisted thoracic surgical approaches to a robotic platform. This has been the source of much of the discussion at our specialty meetings and congresses. This revolution into robotic surgery has forced many surgeons to adopt an entirely new way of operating. Although sometimes difficult, transition to this platform has allowed more patients access to minimally invasive procedures to treat some of the most common diseases we see, name...

Calling All Budding Innovators and Surgeon Scientists to Thoracic Surgery
Innovators and surgeon scientists: where would we be without those thought leaders in surgery? The excitement of hearing surgical innovators share their experiences should be infectious. At the 2023 American Surgical Association, President Diana Farmer, UC Davis Chair of Surgery, presented the path to her landmark Management of Myelomeningocele Study (MOMS) trial of prenatal versus postnatal repair of spina bifida. On the April 2023 episode of Society of Thoracic Surgeons “Same Surgeon, Different Light” podcast, Dr Bartley Griffith, University of Maryland Distinguished Professor of Surgery, discussed his first successf...
