Non-Vitamin K Antagonist Oral Anticoagulant vs Warfarin for Post Cardiac Surgery Atrial Fibrillation.
CONCLUSIONS: NOACs are associated with a reduction in post-operative LOS, without excess bleeding or other short-term complications, compared to warfarin. These findings support the broader use of NOACs as a safe alternative to warfarin in patients with post-cardiac surgery AF at elevated stroke risk and acceptable bleeding risk. The Predictive Value of Inflammatory Biomarkers in Esophageal Anastomotic Leaks.
-Lartigue J Abstract BACKGROUND: The aim of this study is to investigate the role of the neutrophil-to-lymphocyte ratio (NLR) in predicting anastomotic leak (AL) after esophagectomy for esophageal adenocarcinoma. METHODS: This is a retrospective analysis of a prospectively collected database from the McGill University Health Center. It includes all patients with esophageal adenocarcinoma who underwent esophagectomy between 2005-2016. Patients with chronic infections, concurrent active malignancies and autoimmune conditions were excluded. NLR values were obtained on postoperative days (POD) 0, 1, 2, and 3. Rec...

Management of partial anomalous pulmonary venous return in lung transplantation.
We present the case of a patient who underwent bilateral lung transplantation for idiopathic pulmonary arterial hypertension with coexisting partial anomalous pulmonary venous return and tracheal bronchus. The hypoplastic and low-positioned left atrial orifice caused by abnormal reflux of the right upper pulmonary vein and high-positioned right upper lobe bronchus made right anastomosis challenging. To prevent excessive tension on left atrial anastomosis, the donor's right main bronchus was anastomosed to the recipient's bronchus intermedius, which resulted in successful anastomosis and an uneventful postoperative course. ...

Lobectomy with artery reconstruction and pneumonectomy for NSCLC: a propensity score weighting study.
Abstract BACKGROUND: The treatment of NSCLC is based, when suitable, on surgical resection. Pneumonectomy has been considered the standard surgical procedure for locally advanced lung cancers but it is associated with high mortality and morbidity rates. Reconstruction of the pulmonary artery, associated with parenchyma sparing techniques, is meant to be an alternative to pneumonectomy. METHODS: This retrospective single-centre study is based on a detailed and comprehensive analysis of the clinical and oncological data of patients treated between 2004 and 2016 through pneumonectomy or lobectomy with reconstruc...

Successful handmade monobloc aorto-mitral valve replacement for extensive aortic root abscess.
Abstract Aortic root abscess is one of the most catastrophic complications of aortic valve endocarditis. Complete debridement is recommended regardless of the kind of infected lesion. A 37-year-old man with extensive aortic root abscess due to prosthetic aortic valve endocarditis was surgically treated. The main lesion was the aorto-mitral continuity extending to the commissure between the left and right coronary cusps. After debridement, the aortic annulus underneath the left coronary artery was reconstructed using a handmade aorto-mitral monobloc valve without aortic annuloplasty. This valve is required for the ...

Long-term Outcomes of Aortic Valve Replacement with Aortic Homograft: 27 Years Experience.
Abstract BACKGROUND: Aortic homografts have been used in young patients requiring aortic valve replacement(AVR). Currently, they are generally reserved for aortic valve endocarditis with or without root abscess, however, longitudinal data are lacking. Our aim was to assess the long-term safety and durability of homograft implantation. METHODS: All adult patients undergoing aortic homograft implantation at a single institution from 1992-2019 were included. Outcomes of interest included all-cause mortality and AV reoperation, studied over a median follow-up duration of 19 years. RESULTS: A total of 252 pat...

Snare-Assisted Delivery for Ascending Endovascular Repair of Acute Type A Dissection.
Abstract Endovascular stenting of the ascending aorta has been described for various aortic pathologies including Type A dissection in patients who are prohibitive risk for open surgery. Endovascular treatment of ascending aortic pathology poses unique anatomic and technical challenges related to aortic morphology. Herein, we present a novel technique of endovascular ascending stent graft delivery from a transfemoral approach for treatment of an acute Type A dissection, utilizing a snare-guided deployment technique to facilitate favorable device deployment. Closure of a Thoracic Aortic Graft Pseudoaneurysm with an Amplatzer ™ Atrial Septal Defect Occluder.
We describe the case of a young man seven weeks post-operative from repair of a Stanford type A aortic dissection who developed an expanding pseudoaneurysm of the proximal graft anastomosis. Due to the morbidity and mortality associated with re-operation, an interdisciplinary team of interventional cardiologists and cardiothoracic surgeons implanted an Amplatzer™ atrial septal defect occluder device in a hybrid operating room, successfully excluding the defect from the true lumen of the aorta. This case highlights the utility of a team approach and creative thinking for the treatment of a pseudoaneurysm in a high-ris...

Paraplegia after valvular surgery following remote thoracoabdominal aortic aneurysm repair.
Abstract Neurological disorders are the most unpredictable and feared complications after open surgery or endovascular aortic repair. Paraplegia due to spinal cord injury is well known after thoracoabdominal aortic surgery, but not after valvular surgery. We herein present a case of paraplegia after mitral and tricuspid valve surgery in a patient with a history of surgery involving the thoracoabdominal and abdominal aorta. The paraplegia was likely caused by temporary postoperative hypotension as low as 40 mmHg for more than 10 minutes with decreased spinal perfusion in the intensive care unit. PMID: 33421387...

Direct versus side graft cannulation from the right axillary artery in thoracic aortic surgery.
CONCLUSIONS: Cannulating the right axillary through a vascular prosthetic graft reduces cannulation-related complications such as iatrogenic axillary artery dissection, and lowers stroke rates. Computed Tomography during Bronchoscopic Lung Splinting for Atelectasis.
Abstract A 64-year-old man experienced persistent atelectasis of the right lung following right upper lobectomy. To simultaneously visualize the airways and lung parenchyma in real-time, chest computed tomography was performed while pneumatically splinting the lung open via insufflation through the working channel of a bronchoscope. The bronchi were patent but peripheral consolidations within the remaining right lung were visualized, representative of pneumonia. The patient fully recovered with antimicrobial therapy. CT during bronchoscopic pneumatic lung splinting is an advanced diagnostic for the investigation o...
