Pre-operative veno-venous ECMO for D-Transposition with severe pulmonary hypertension in a newborn.
We report the case of a newborn with transposition of great arteries and ventricular septal defect associated with severe pulmonary hypertension. Profound hypoxaemia despite successful balloon atrial septostomy and conventional supportive measures with mechanical ventilation, inhaled nitric oxide, and inotropes led to the use of veno-venous extracorporeal membrane oxygenation to rapidly stabilized the child pre-operatively. Different from most reported cases on this scenario, we intentionally opted for a veno-venous mode of support despite the presence of circulatory compromise. Bilateral lobar transplants using one donor for two small size recipients.
We present here a case where one donor was used for bilateral lobar transplantations in two high-risk patients. Coordination between the graft preparation at the back table and the two concomitant LTx teams were necessary to minimize the ischemic injury of the grafts as well as to plan for adequate vascular and bronchial cuffs for both implantations. Omental flap for bronchopleural fistula post pneumonectomy and aorta replacement.
We report the case of a 56-year-old man treated with omental flap for BPF after pneumonectomy along with descending aorta replacement. He underwent left pneumonectomy with combined resection of the descending aorta, followed by replacement with prosthetic graft after the diagnosis of lung cancer, cT4N1M0 Stage IIIA. He had BPF postoperatively and underwent an omental flap plombage following unsuccessful repair using the latissimus dorsi muscle. He did not have BPF recurrence or aortic graft infection. An omental flap is a useful option for treating BPF with intrathoracic prosthetic graft. PMID: 31586619 [PubMed - as s...

Right Ventricle Dependent Coronary Circulation: Location of Obstruction is Associated with Survival.
Abstract BACKGROUND: Pulmonary atresia with intact ventricular septum (PAIVS) with right ventricle dependent coronary circulation (RVDCC) carries suboptimal outcomes primarily due to cardiac ischemia. We hypothesize clinical outcomes are affected by the level of coronary obstruction, a surrogate for vulnerable myocardium. METHODS: We conducted a single-institution retrospective analysis of all neonates with PAIVS with RVDCC from 1995-2017. RVDCC was defined as the presence of any coronary-cameral fistula with coronary obstruction proximal to the fistula and angiographic evidence of RV perfusion of the myocard...
