Emergency department use after outpatient thyroidectomy across three states
Although outpatient thyroidectomy has become common, few large-scale studies have examined post-thyroidectomy emergency department use, readmission, and encounters not resulting in readmission, known as “treat-and-release” encounters. We evaluated post-outpatient thyroidectomy emergency department use and readmission and characterized associated factors. (Source: Surgery)

Dr T.K. Pandian (St. Louis, MO): T.K. Pandian, St. Louis, Missouri. To provide some context, deciding how much parathyroid tissue to leave behind is a difficult question to address. I commend you all for protocolizing this and coming up with some concrete recommendations for those of us that do parathyroid surgery. How did you determine this size remnant, 75 –100 mg? The historical teaching has always been 50 mg. Where did the 75–100 come from? In addition, please tell me a little bit about your intraoperative PTH goal of 150–250? (Source: Surgery)

Prophylactic mesh does not prevent parastomal hernia in long-term: Meta-analysis and trial sequential analysis
Previous randomized clinical trials, systematic reviews, and meta-analyses evaluating parastomal hernia prevention with mesh placement during end colostomy formation have reported contradictory results. This review aimed to assess the efficacy of this strategy in long-term follow-up according to the latest available data. (Source: Surgery)

Tumor Size Reduction and Serum Carbohydrate Antigen 19-9 Kinetics After Neoadjuvant FOLFIRINOX in Patients With Pancreatic Ductal Adenocarcinoma
Changes in tumor size and serum carbohydrate antigen 19-9 are commonly reported markers used to assess response to neoadjuvant therapy in pancreatic ductal adenocarcinoma. We evaluated the impact of the percentual tumor size reduction and carbohydrate antigen 19-9 kinetics on resectability and response to neoadjuvant FOLFIRINOX. (Source: Surgery)

Forecasting The Impact of the 2023 Current Procedural Terminology Coding Changes On Ventral Hernia Work Relative Value Units. A Cross-Sectional Study
In January 2023, significant changes were implemented to ventral hernia repair Current Procedural Terminology codes, with new codes replacing previous codes. The new codes were assigned a 0-day global period. The impact of these changes on clinical productivity remains unclear. Our objective was to forecast the impact of Current Procedural Terminology changes on ventral hernia-related work relative value units using historical data. (Source: Surgery)

Social determinants as predictors of resection and long-term mortality in Black patients with non-small cell lung cancer
Minorities diminished returns theory posits that socioeconomic attainment conveys fewer health benefits for Black than White individuals. The current study evaluates the effects of social constructs on resection rates and survival for non-small cell lung cancer (NSCLC). (Source: Surgery)

American Association of Endocrine Surgeons recorder ’s note (January 2024 issue)
This issue of Surgery, like past years, includes the proceedings of our annual American Association of Endocrine Surgeons (AAES) meeting from April 2023 in Birmingham, Alabama. It features the excellent scientific work of our members with the transcribed discussions of each presented paper, which are in many ways even more illustrative than the work itself. This issue also contains the ‘AAES Position Statement on Selected Endocrine Surgery Billing Codes and Procedures: Addressing Gaps in the Current Coding Paradigm’ and an expanded section on innovation and technology. Dr Matthew Nehs (Boston, MA): These tumors are highly glycolytic. The most aggressive subtypes may be producing cortisol to augment their growth. Did you look at glucose levels? Did you look at Hg A1C, or the presence of diabetes amongst the different hormone secreting subtypes? If so, was there a difference in the groups? (Source: Surgery)

Ashok Shaha (New York, NY): Wonderful presentation. Surgeons from Italy and Argentina have also reported similar findings. Patients will often feel like they are being strangulated. The proposed theory is scarring and fibrosis. There may be parasympathetic dysfunction near the cricopharyngeus muscle, which occurs during the dissection of the thyroid. This leads to cervical esophageal motility disturbance. You may want to look into this to improve your study and results. (Source: Surgery)

Christopher McHenry (Cleveland, OH): It is unfortunate but accurate that patients often use the emergency department (ED) as their primary care option. This is a very difficult problem that has been around for a long time. It also contributes to the increased costs of health care. My question is, what can we do pre- and post-operatively to change or mitigate this problem? Should we give a direct number for the patients to call before they go to the ED? (Source: Surgery)

Patient characteristics and procedural and safety outcomes of percutaneous transesophageal gastro-tubing: A nationwide database study in Japan
Percutaneous transesophageal gastro-tubing (PTEG) is an alternative interventional procedure in cases where gastrostomy is not feasible. However, the safety and complication rates of PTEG have not yet been evaluated. We aimed to describe the characteristics of patients who underwent PTEG and investigate complications using a nationwide Japanese inpatient database. (Source: Surgery)

Extended antibiotic therapy is associated with a lower rate of clinically relevant postoperative pancreatic fistula after pancreatoduodenectomy in intermediate- and high-risk patients: A single-institution analysis
We report outcomes after pancreatoduodenectomy in patients at increased risk for clinically relevant postoperative pancreatic fistula who received extended antibiotic therapy compared to those who received standard perioperative antibiotics (single dose before incision). (Source: Surgery)

Disparities in access to high-volume parathyroid surgeons in the United States: A call to action
Parathyroidectomy by a high-volume surgeon is associated with a reduced risk of perioperative complications and of failure to cure primary and secondary hyperparathyroidism. There are limited data on disparities in access to high-volume parathyroid surgeons in the United States. (Source: Surgery)
