Dr T.K. Pandian (St. Louis, MO): T.K. Pandian, St. Louis, Missouri. To provide some context, deciding how much parathyroid tissue to leave behind is a difficult question to address. I commend you all for protocolizing this and coming up with some concrete recommendations for those of us that do parathyroid surgery. How did you determine this size remnant, 75 –100 mg? The historical teaching has always been 50 mg. Where did the 75–100 come from? In addition, please tell me a little bit about your intraoperative PTH goal of 150–250?
