Malpractice litigation after thyroid surgery: What factors favor surgeons?
Litigation impacts physicians financially, reputationally, and professionally. Although thyroid surgery has favorable patient outcomes, litigation persists. We aimed to characterize malpractice claims after thyroidectomy and investigate which factors favor physicians. (Source: Surgery)

Research training as an integral part of surgical resident education: Central Surgical Association presidential address
It has been an honor to serve as President of the Central Surgical Association during the past year. I have enjoyed working with many of the past presidents of the Central Surgical Association in various societies, including the Central Surgical Association, the American Surgical Association, the American College of Surgeons, and the American Board of Surgery (Figure  1). These colleagues have had a major influence on my career in academic surgery. I would also like to acknowledge and thank the most recent past presidents and our current president-elect of the Central Surgical Association; these individuals are thoughtful...

How often are intrathyroidal parathyroid glands a cause of primary hyperparathyroidism, and how should they be managed?
The purpose of our study was to determine the frequency and management of intrathyroidal parathyroid glands in patients with primary hyperparathyroidism and evaluate whether intrathyroidal parathyroid glands were more often superior or inferior glands. (Source: Surgery)

Discussion
Allan Siperstein (Cleveland, OH): Having served on our medical-legal committee for a decade, I am very familiar with this whole area, and medical centers will only go to court if they feel that they can win. Most cases are settled. You present data on the court rulings, but do you have data on all the cases that were settled? (Source: Surgery)

Combined hepatocellular-cholangiocarcinoma compared to hepatocellular carcinoma and intrahepatic cholangiocarcinoma: Different survival, similar recurrence
Combined hepatocholangiocarcinoma is a rare cancer with a grim prognosis composed of both hepatocellular carcinoma and intrahepatic cholangiocarcinoma morphologic patterns in the same tumor. The aim of this multicenter, international cohort study was to compare the oncologic outcomes after surgery of combined hepatocholangiocarcinoma to hepatocellular carcinoma and intrahepatic cholangiocarcinoma. Surgical management of 1- to 2-cm neuroendocrine tumors of the appendix: Appendectomy or right hemicolectomy?
The surgical management of 1- to 2-cm neuroendocrine tumors of the appendix is an area of debate. We analyzed the clinical outcomes of appendectomy and compared them to right hemicolectomy. (Source: Surgery)

Normocalcemic hyperparathyroidism: Intervention to differentiate primary from secondary hyperparathyroidism
Normocalcemic hyperparathyroidism can occur, but surgery should not be considered until common etiologies for secondary hyperparathyroidism are comprehensively excluded. Calcium deficiency is an underrecognized cause of normocalcemic parathyroid hormone elevation, and we aim to determine if the implementation of a preoperative calcium challenge can be used to reduce unnecessary parathyroidectomy. Patients with prediabetes improve insulin resistance after surgery for primary hyperparathyroidism
The objective of this study was to evaluate the evolution of insulin resistance at 12 months after parathyroidectomy for primary hyperparathyroidism according to the preoperative severity of glucose metabolism abnormalities. (Source: Surgery)

The unfunded grant, now what? Advice, approach, and strategy
Grant writing takes significant time and effort and often may be elusive, especially on a first attempt. After the rejection of a grant, many investigators face a dilemma regarding the best next steps. In this article, we discuss the options of revision versus resubmission and how to navigate these decisions. (Source: Surgery)

The effect of amantadine on acute cognitive disability after severe traumatic brain injury: An institutional pilot study
Amantadine is used in the post-acute care setting to improve cognitive function after a traumatic brain injury. Its utility in the acute postinjury period is unknown. In this pilot study, we sought to examine the effect of amantadine on short-term cognitive disability among patients with a severe traumatic brain injury and hypothesized that patients receiving amantadine would have a greater improvement in disability throughout their acute hospitalization. Inpatient opioid use and pain control after robotic versus laparoscopic sleeve gastrectomy
During the past decade, the proportion of bariatric surgery performed robotically has been rising. However, the clinical benefits of the robotic approach over laparoscopy are uncertain. One area in need of further investigation is pain control after robotic versus laparoscopic bariatric surgery. (Source: Surgery)

Discussion
David Schneider (Madison, WI): We know that patients with secondary hyperparathyroidism can have an elevated parathyroid hormone (PTH) for 6 months or longer after correction of the secondary cause. Based on this, how long do you monitor these patients? I still worry about patients with a slightly increased PTH and an unsuppressed Ca. How long would you monitor these patients? (Source: Surgery)

Discussion
Dr Maggie Brandt (University of Oklahoma): In your amantadine group, you indicated it was physician-specific. Is there some sort of protocol in your institution, such as when you do or do not administer amantadine? Did you look at any of the other medications, like the atypical antipsychotic meds? If they were given amantadine and stopped amantadine, did you put them in the amantadine group, or were they included in the no-amantadine group? Was age a factor? If a patient was over a certain age or under a certain age, could you tell if that had any effect either on whether they received amantadine or what their outcome was?...
