Acupuncture as an Adjunct Treatment to Cognitive-Behavioral Therapy for Insomnia
Cognitive-behavioral therapy for insomnia (CBT-I) is the main recommended treatment for patients presenting with insomnia; however, the treatment is not equally effective for all, and several factors can contribute to a diminished treatment response. The rationale for combining CBT-I treatment with acupuncture is explored, and evidence supporting its use in treating insomnia and related comorbidities is discussed. Practical, regulatory, and logistical issues with implementing a combined treatment are examined, and future directions for research are made. Growing evidence supports the effectiveness of acupuncture in treatin...

Adjunct Interventions to Cognitive Behavioral Therapy for Insomnia
Cognitive Behavioral Therapy for Insomnia (CBT-I) has been consistently demonstrated to be an efficacious treatment of insomnia1 and is widely acknowledged as the first-line treatment for insomnia.2,3 However, because CBT-I essentially consists of cognitive restructuring and behavior modification that require patient engagement and their active participation, it has inherent limitations. First, suboptimal adherence to CBT-I components is a known limitation and reduces its impact.4 Second, a considerable proportion of patients drop out, with attrition rates in sleep clinics ranging from 10% to 40%. Continuous Positive Airway Pressure Use for Obstructive Sleep Apnea in Pediatric Patients
Pediatric obstructive sleep apnea is becoming more common and better diagnosed and treated. It is seen in children with obesity, genetic disorders, neuromuscular disorders, and congenital malformations. After adenotonsillectomy, continuous positive airway pressure (CPAP) is a major mode of treatment. CPAP is traditionally initiated by titrating in the laboratory. However, auto-CPAP titration in the home environment is becoming more accepted as an option. When children are adherent to CPAP, there are significant benefits to treatment, both short-term and long-term. The short-term benefits include improved behavior, focus, a...

Clinical Decision-making for Continuous Positive Airway Pressure Mask Selection
Continuous positive airway pressure is the gold standard treatment for obstructive sleep apnea. Different interfaces with distinct characteristics, advantages, and disadvantages are available, which may influence long-term adherence. Oronasal masks have been increasingly used. However, recent evidence suggest that nasal masks are more effective when continuous positive airway pressure is used to treat obstructive sleep apnea. The main objective of this review is to describe the basis for the selection of the interface for the treatment of obstructive sleep apnea with continuous positive airway pressure. The Relationship Between Epilepsy, Obstructive Sleep Apnea, and Treatment Outcome
Both epilepsy and obstructive sleep apnea are common conditions and hence frequently coexist in a given patient. A complex bidirectional relationship exists between the 2 conditions where the presence of one affects the other. Treatment of obstructive sleep apnea with continuous positive airway pressure may improve seizure control in medically refractory epilepsy patients, leading to improved quality of life. Understanding of this complex relationship between epilepsy, sleep, and sleep disorders such as obstructive sleep apnea continues to evolve. (Source: Sleep Medicine Clinics)

Continuous Positive Airway Pressure Therapy for Sleep Apnea: A Newly Controversial Topic
Until recently, treatment for obstructive sleep apnea (OSA) was not a controversial topic. Untreated OSA is associated with increased rate of myocardial infarction, stroke, hyperlipidemia, hypertension, diabetes, and arrhythmias, including atrial fibrillation and pulmonary hypertension, to mention just a few of its most significant associated conditions. Because continuous positive airway pressure (CPAP) is a safe and effective treatment to reduce sleep apnea, it has been assumed for decades to reduce associated cardiovascular risks.
