Cervical and systemic innate immunity predictors of HIV risk linked to genital herpes acquisition and time from HSV-2 seroconversion
Conclusions A combination of systemic immunoinflammatory and cervical immunosuppressed states predicts HSV-2 acquisition. A persistently suppressed innate immunity during incident HSV-2 infection may add to the increased HIV-1 susceptibility. (Source: Sexually Transmitted Infections)

Determinants of adherence to daily PrEP measured as intracellular tenofovir diphosphate concentrations over 24 months of follow-up among men who have sex with men
Conclusions Overall, TFV-DP concentrations were high among MSM using dPrEP, indicating excellent adherence. Especially older participants, those who reported condomless anal sex with a casual partner prior to PrEP initiation and those who used an app with visualised feedback showed higher levels of adherence. As TFV-DP concentrations had decreased slightly at 2 years of PrEP use when compared with 1 year, we emphasise the importance of adherence counselling to those who continue using PrEP. Trial registration number NL5413. (Source: Sexually Transmitted Infections)

Towards the prevention of sexually transmitted infections (STIs): Healthcare-seeking behaviour of women with STIs or STI symptoms in sub-Saharan Africa
Conclusion The findings of the study call for strengthening of policies, programmes, and interventions geared towards improving thehealthcare-seeking behaviour of women with STIs, taking into consideration the factors identified in this study. (Source: Sexually Transmitted Infections)

Race to address sexual health inequalities among people of Black Caribbean heritage: could co-production lead to more culturally appropriate guidance and practice?
People of Black Caribbean heritage in the UK experience a disproportionate burden of STIs. This has persisted for decades. Notably, rates of diagnosed chlamydia and gonorrhoea in England among those of Black Caribbean ethnicity were over five times and nearly six times higher, respectively, compared with rates among White ethnic groups, in 2021.1 Such health inequalities are widely recognised among racially minoritised communities2; for example, the highest COVID-19 mortality rates were observed among non-White racial groups.3 Generally, poverty contributes to poorer health outcomes,2 and people of Black Caribbean heritage...

Research news in clinical context
What causes persistent hepatitis B virus (HBV) DNA detection despite tenofovir-containing antiretroviral therapy (ART)? Investigators analysed the occurrence of HBV DNA detection (≥20 IU/mL) among 222 people with HIV/HBV coinfection who started any tenofovir-containing ART, achieved HIV RNA suppression of <200 copies/mL and stayed in follow-up for a median of 8.4 years. After 2 years of ART, 61 (27%) of the 222 people with HIV/HBV had detectable HBV DNA, including 6 (10%) of 61 with HBV DNA of >2000 IU/mL. In multivariable analysis, the odds of HBV DNA detection were reduced with low baseline HBV DNA levels, basel...

Returning home sampling kits for STI and HIV testing in people using a digital health HIV-PrEP pathway (PrEP-EmERGE)
Current standard of care for people using oral HIV-pre-exposure prophylaxis (PrEP) involves face-to-face clinic appointments for HIV, syphilis, gonorrhoea and chlamydia (STI) testing every 3 months.1 Providing more convenient clinical pathways can help optimise HIV-PrEP as a prevention strategy.2 PrEP-EmERGE is an mHealth application (app) and digital pathway co-designed by users and clinicians to support and empower people using HIV-PrEP, and reduce the number of face-to-face clinic appointments.3 PrEP-EmERGE aims to support people using HIV-PrEP by providing users with secure access to their results (recent and historic)...

Mpox proctitis as a likely predisposing factor for chlamydial perihepatitis in a male patient
We report a case of chlamydial perihepatitis in a male patient, occurring 1 month after Mpox and associated with the unusual LGV ST23 strain. Our case suggests that rectal Mpox lesions may facilitate chlamydial dissemination. (Source: Sexually Transmitted Infections)

MPox virus: an unusual aetiology of peritonitis
We describe a rare case of severe disseminated monkeypox (MPox) virus infection complicated by peritonitis in a 44-year-old man living with well-controlled HIV. The patient was successfully treated with tecovirimat without requiring surgery. MPox should be considered in the differential diagnosis of non-bacterial peritonitis in patients at risk of infection. Case of apparent mpox reinfection
We present an apparent second episode of mpox (monkeypox) genital ulcerative disease in a non-immunosuppressed MSM (man who has sex with men) patient who had completely recovered from a primary mpox infection 4 months previously. The patient had also received a complete two-dose course of smallpox vaccination between the two presentations. This case highlights the importance of continuing to include mpox in the differential diagnoses for individuals presenting with genital or mucosal ulceration, regardless of assumed immunity derived from prior infection or vaccination. High-grade and low-grade HPV-induced urethral lesions treated by CO2 laser under colposcopy
Conclusions HSIL was present in the urethra without being able to define specific clinical criteria. Treatment with a CO2 laser under colposcopy with a meatus spreader is a simple surgical procedure with high efficiency and few complications that could prevent the risk of HPV-induced carcinoma. Insufficient PrEParation: an assessment of primary care prescribing habits and use of pre-exposure prophylaxis in patients at risk of HIV acquisition at a single medical centre
Conclusions This study highlights intervention points in the HIV prevention cascade warranting attention in order to achieve the 2025 Ending the HIV Epidemic in the U.S. target for PrEP coverage. These include increasing provider adherence to prescribing guidelines and reducing the logistical barriers to prescribing. (Source: Sexually Transmitted Infections)

Measuring quality STI care among adolescent female primary care patients in Philadelphia
Conclusions Local application of an STI Care Continuum identified STI testing, retesting and HIV testing as areas for improvement. The development of an STI Care Continuum identified novel measures for monitoring progress towards National Strategic indicators. Similar methods can be applied across jurisdictions to target resources, standardise data collection and reporting and improve STI care quality. (Source: Sexually Transmitted Infections)

Prevalence of STIs and peoples satisfaction in a general population STI testing site in Bern, Switzerland
Conclusion We found many asymptomatic bacterial STIs requiring treatment. Offering easily accessible STI testing and counselling proved successful as shown by increasing rates of attendance and high levels of satisfaction. (Source: Sexually Transmitted Infections)

How did the COVID-19 pandemic affect access to condoms, chlamydia and HIV testing, and cervical cancer screening at a population level in Britain? (Natsal-COVID)
Conclusions Our study suggests differential access to key primary and secondary STI/HIV prevention interventions continued during the first year of the COVID-19 pandemic. However, there was not strong evidence that differential access has changed during the pandemic when compared with 2010–2012. While the pandemic might not have exacerbated inequalities in access to primary and secondary prevention, it is clear that large inequalities persisted, typically among those at greatest STI/HIV risk. Prevalence of macrolide and fluoroquinolone resistance-associated mutations in Mycoplasma genitalium in metropolitan and overseas France
Conclusion This study reports the high prevalence of macrolide and fluoroquinolone resistance-associated mutations in M. genitalium in metropolitan France and highlights the contrast with low prevalence in overseas France. In metropolitan France, macrolide resistance-associated mutation prevalence was three times higher in men than in women, which was likely to be driven by the proportion of men who have sex with men. This suggests that gender and sexual practice should also be taken into account for the management of M. genitalium infections. (Source: Sexually Transmitted Infections)
