Detecting anal human papillomavirus infection in men who have sex with men living with HIV: implications of assay variability
Conclusions Both methodologies identified a high prevalence of anal HPV infection and multiple HPV infections in MSM living with HIV, showing a moderate overall agreement between them. Either total HPV detection or type-specific HPV16/18 detection together with a threshold ≥atypical squamous cells of undetermined significance for abnormal cytology showed an acceptable diagnostic accuracy. Analysis of cervical HPV infections among unvaccinated young adult women to inform vaccine strategies in this age group: the Costa Rica HPV Vaccine Trial
Conclusion Young adult women may benefit from HPV vaccination if newly acquired vaccine-preventable oncogenic infections lead to cervical precancer and cancer. HPV vaccination targeting this population may provide additional opportunities for primary prevention. Trial registration number NCT00128661. (Source: Sexually Transmitted Infections)

Sexual behaviours and sexual health among middle-aged and older adults in Britain
Conclusions The use of these four classes can aid in improved targeting of tailored sexual health services to improve sexual function, sexual satisfaction, reduce distress and risky behaviours among middle-aged and older adults. These services should be inclusive of the disabled community. (Source: Sexually Transmitted Infections)

Pre-exposure prophylaxis and bacterial sexually transmitted infections (STIs) among gay and bisexual men
Conclusions Our study adds to the growing awareness that PrEP use among GBM may be associated with bacterial STIs because PrEP users have more anal sex partners and are more likely to engage in CAS. The results underscore the importance of providing effective STI counselling and regular testing to PrEP users, adapting PrEP care and related STI testing to individual needs, and the need for effective prevention strategies for bacterial STIs. Douching cessation and molecular bacterial vaginosis: a reanalysis of archived specimens
Conclusions In this pilot study, douching cessation was not associated with major changes in vaginal microbiota. Douching cessation alone may not durably shift the vaginal microbiota and additional interventions may be needed to restore optimal vaginal microbiota among those who douche. Can we screen less frequently for STI among PrEP users? Assessing the effect of biannual STI screening on timing of diagnosis and transmission risk in the AMPrEP Study
Conclusion Reducing the STI screening frequency to biannually among PrEP users will likely result in delayed diagnoses, potentially driving onward transmission. Although determinants for asymptomatic STIs were identified, predictive power was low. (Source: Sexually Transmitted Infections)

Research news in clinical context
Published in STI: is a diagnosis of rectal gonorrhoea a predictor of HIV risk in men who have sex with men? Incidence of rectal gonorrhoea in men who have sex with men (MSM) is regarded as an indicator of increased risk of HIV infection,1 but the relationship may be modified by effective antiretroviral treatment (ART) and HIV pre-exposure prophylaxis (PrEP) programmes. Between 2011 and 2018 (prior to wide implementation of HIV PrEP), among 541 056 HIV-negative MSM attending sexual health services in England, incidence of rectal gonorrhoea increased from 3.45 to 11.13 per 100 person-years, whereas HIV incidence declined fro...

Correspondence on 'Lymphogranuloma venereum mimicking squamous cell carcinoma by Sara Tormo-Mainar et al
A man living with HIV receiving virologically suppressive antiretroviral therapy with preserved CD4 cell count presented with a 1-month history of a painful, large verrucous lesion on the left buttock (figure 1A). The differential diagnosis included mainly verrucous squamous cell carcinoma and lymphogranuloma venereum (LGV). Chronic herpes simplex virus infection or Buschke-Löwenstein tumour was considered less likely. A biopsy demonstrated pseudoepitheliomatous hyperplasia and dermal inflammatory infiltrates (figure 1B). Molecular testing detected Chlamydia trachomatis serovar L2. Twenty-one days of doxycycline (1...

Greening up sexual health services
Climate change has a negative impact on sexual and reproductive health worldwide, through mechanisms such as healthcare disruption, food insecurity leading to sex work and child marriage, and increased gender-based violence following extreme weather events.1 Sexual and reproductive healthcare has an important role to play in building climate resilience, for example, through facilitating pregnancy spacing, but healthcare systems also contribute significantly to global emissions.2 Our integrated sexual health, contraception and HIV department formed a ‘green team’ aiming to increase environmental sustainability w...

Acceptability and feasibility of sexual health screening in psychiatry inpatients
Parity in mental and physical health should include at least adequate physical healthcare in psychiatric patients, including oft-neglected sexual health. Several studies suggest an increased prevalence of STIs in people with serious mental illness.1–4 Multiple factors may account for this, and standard STI prevention interventions are not always helpful.5 We sought to understand whether psychiatric inpatients would welcome opportunistic sexual health assessments and the practicalities of delivering the assessments. Eligible patients were those admitted to general adult psychiatry wards (age range 18–65) at the ...

Hepatitis A and B vaccine uptake and immunisation among men who have sex with men seeking PrEP: a substudy of the ANRS IPERGAY trial
Vaccination against hepatitis A virus (HAV) and hepatitis B virus (HBV) is recommended in men who have sex with men (MSM). We assessed HAV and HBV vaccine uptake in the non-immune participants and their immunisation during follow-up of the ANRS IPERGAY (Intervention Préventive de l'Exposition aux Risques avec et pour les Gays) pre-exposure prophylaxis (PrEP) trial. During the ANRS IPERGAY trial among MSM (NCT 01473472), vaccination against HAV and HBV was offered free of charge to all non-immune participants at baseline. We assessed anti-HAV IgGs and anti-hepatitis B surface (HBs) antibodies (Abs) at baseline, 1&nda...
