Time to treat the climate and nature crisis as one indivisible global health emergency
Over 200 health journals call on the United Nations (UN), political leaders and health professionals to recognise that climate change and biodiversity loss are one indivisible crisis and must be tackled together to preserve health and avoid catastrophe. This overall environmental crisis is now so severe as to be a global health emergency. The world is currently responding to the climate crisis and the nature crisis as if they were separate challenges. This is a dangerous mistake. The 28th Conference of the Parties (COP) on climate change is about to be held in Dubai while the 16th COP on biodiversity is due to be held in T...

Research news in clinical context
Combining the HPV genotype with epidemiological and clinical parameters markedly improves prediction of cervical cancer Researchers in China explored the added value of HPV genotyping in predicting cervical intraepithelial neoplasia (CIN). Data from 21 720 women (median age 50 years) with high-risk HPV formed a training dataset (n=14 553; 349 with CIN3+; 673 with CIN2+) and a validation dataset (n=7167; 167 with CIN3+; 228 with CIN2+). A machine learning model that added HPV genotypes to epidemiological factors (demographic characteristics, medical history, menstrual status, sexual behaviour factors, family history of canc...

Sexual Function and Well-being Special Interest Group: what is in a name?
The Sexual Function and Well-being Special Interest Group (SIG) of the British Association for Sexual Health and HIV aims to improve the quality of care given to patients presenting to sexual health clinics with sexual dysfunction and provide a forum for education, training and research. It recently changed its name to reflect the positive evolution from a problem-orientated sexual dysfunction SIG. The emphasis on sexual function aligns with the WHO definition of sexual health as ‘a state of physical, emotional, mental and social well-being about sexuality; not merely the absence of disease dysfunction or infirmity&r...

HIV-1 DNA drug resistance testing to guide successful suppression of low-level viraemia: a case report
A treatment-naïve 10-year-old female patient living with HIV-1 infection started antiretroviral therapy (ART) in April 2018 with zidovudine+lamivudine+nevirapine, which was changed 19 months later to zidovudine+lamivudine+efavirenz. The HIV-1 RNA load ranged between 55 and 420 copies/mL between February 2020 and May 2021, and was 485 copies/mL in January 2022. At this time, the total HIV-1 DNA load in peripheral blood was 2260 copies/million white blood cells (WBCs) as measured with a HIV-1 DNA quantification kit (Supbio, Guangzhou, China). Genotypic drug resistance testing (pol region amplification and Sanger sequ...

PrEP2U: a novel community partnership HIV pre-exposure prophylaxis clinic
Broadening the access of HIV–pre-exposure prophylaxis (HIV-PrEP) prevents HIV transmission.1 In Brighton, we partnership with The Terrence Higgins Trust to deliver a community-based HIV-PrEP service (PrEP2U).2 PrEP2U operates weekly, offering comprehensive STI/HIV testing, HIV-PrEP monitoring and hepatitis A & B and human papilloma virus vaccinations. We aimed to explore whether PrEP2U is providing HIV-PrEP to a different population compared with our clinic. Eighty-two individuals attended PrEP2U between August and December 2022, 66 (80%) identified as cis-male, 5 (6%) cis-female, 5 (6%) trans-feminine, 2 (2%) tr...

A case of chronic bacterial prostatitis due to Mycoplasma genitalium
We describe the case of a 42-year-old man with recurrent urinary tract infections since 2018 who developed chronic prostatitis despite several and prolonged antibiotic courses. Multiparametric prostatic magnetic resonance showed peripheral inflammatory alterations. A 4-glass Meares-Stamey test detected MG in the third voided bladder (VB3) sample. Moxifloxacin 400 mg daily for 28 days resulted in sustained clinical and microbiological cure. (Source: Sexually Transmitted Infections)

Timing of HPV vaccination as adjuvant treatment of CIN2+ recurrence in women undergoing surgical excision: a meta-analysis and meta-regression
Conclusions The outcome of this meta-analysis and meta-regression convincingly showed the beneficial effect of post-excisional HPV vaccination against CIN2+ recurrence. Studies published to date have been unable to determine whether or not vaccination, completed or initiated before conisation, would be associated with more favourable results. Differential uptake and effects of digital sexually transmitted and bloodborne infection testing interventions among equity-seeking groups: a scoping review
Conclusion Evidence of health equity effects of digital STBBI testing remains limited. While digital STBBI testing interventions increase testing across sociodemographic strata, increases are lower among historically disadvantaged populations with higher prevalence of STBBIs. Findings challenge assumptions about the inherent equity of digital STBBI testing interventions, emphasising the need to prioritise health equity in their design and evaluation. High uptake of vaccination against mpox in men who have sex with men (MSM) on HIV pre-exposure prophylaxis (PrEP) in Paris, France
In July 2022, the French health authorities recommended smallpox vaccination (IMVANEX and JYNNEOS) to prevent mpox in population at high risk of exposure. HIV and pre-exposure prophylaxis (PrEP) physicians were informed and involved in the deployment of this vaccination, but there has been no communication to the general public. The current mpox epidemic in France, the rest of Europe and the USA is concentrated among men who have sex with men (MSM), particularly in case of multiple physical and sexual contacts.1 2 Several case series have reported high proportions of people living with HIV and of users of HIV–PrEP.3 ...

Retrospective testing for mpox virus in routine STI screens from men who have sex with men in England, August-October 2022
Conclusions A very low prevalence (2 of 1159, 0.17%) of mpox infection was detected in GBMSM attending SHS in England for asymptomatic NG/CT screening, suggesting that undetected infection in this population was unlikely to be a main driver of transmission. Confirmed mpox cases in the UK declined from over 1100 per month in June and July to 764 cumulatively during the collection period. These data give reassurance that the observed reduction in cases during the collection period was not due to undetected infection or changes in presentation among SHS attendees. Currently, there is insufficient evidence to support routine t...

Egocentric sexual network analysis among gay and bisexual men who have sex with men with and without mpox infection
Conclusions Our findings highlight the role of demographic, epidemiological and sexual network characteristics in the transmission of mpox virus during the outbreak in Spain. These findings have important implications for future prevention efforts. (Source: Sexually Transmitted Infections)

Understanding experiences of potential harm among MSM (cis and trans) using HIV self-testing in the SELPHI randomised controlled trial in England and Wales: a mixed-methods study
Discussion HIVST harms were rare but strategies to link individuals experiencing harms with psychosocial support should be considered for HIVST scale-up. Trial registration number ISRCTN20312003. (Source: Sexually Transmitted Infections)

Impact of time of culture specimen collection on the recovery of Neisseria gonorrhoeae after a positive nucleic acid amplification test
Conclusion Reducing the interval between screening and collection of culture specimens increased N. gonorrhoeae recovery in extragenital samples. Implementing a same-day testing and notification programme increased collection of culture samples and culture yield in our setting, which may help AMR surveillance. Using infection prevalence, seroprevalence and case report data to estimate chlamydial infection incidence
Conclusions Our method can be used to estimate the number of chlamydia infections each year, and thus whether infection incidence increases or decreases over time and after policy changes. Furthermore, our results suggest that clearance via medical intervention may lead to short-term or no seroconversion, and the duration of untreated chlamydial infection may vary with age, underlining the complexity of chlamydial infection dynamics.
