Extracorporeal membrane oxygenation support in the setting of penetrating traumatic injuries
ConclusionIn this study we show penetrating traumatic injuries should not be considered a contraindication to ECMO support, and how ECMO can be a useful treatment strategy in selected patients with these injuries. (Source: Journal of Cardiac Surgery)

Understanding aortic valve repair through Ozaki procedure: A review of literature evidence
ConclusionAV Neo can be a suitable alternative to surgical AV replacement in selected patients. The short- and midterm outcomes are comparable without the need for long-term oral anticoagulation. Long-term follow-up data are required for this novel approach to be widely adopted. (Source: Journal of Cardiac Surgery)

Aortic valve neocuspidization using autologous versus bovine pericardium: Ozaki versus Batista
ConclusionAV Neo offers a means to perform SAVR whilst escaping the prosthetic valve issues. However, the Batista procedure has shown beyond doubt that it can be considered the superior approach for AV Neo over the Ozaki procedure. (Source: Journal of Cardiac Surgery)

Aortic arch replacement and autologous pericardial tracheal patch for an aorto ‐tracheal fistula
We present a 48-year-old man with an aorto-tracheal fistula induced by radiation therapy for tracheal squamous cell carcinoma. He presented with hemoptysis and chest pain and workup revealed the aorta-tracheal fistula between the posterior aortic arch and anterior distal trachea. He was emergently taken to surgery. To our knowledge, this is the first report of an aorto-tracheal fistula successfully treated with a transverse aortic arch replacement and complex tracheal repair using autologous pericardium with an omental buttress. Too big to fail? —An aggressive strategy for a dire problem
AbstractTracheo-innominate fistula (TIF) is a reported complication of tracheostomy that typically presents with a herald bleed. The phenomenon of an aortotracheal fistula has similar pathology and presentation to TIF, but no standard surgical repair. In the manuscript by Musgrove et al. in theJournal of Cardiac Surgery, the authors propose a surgical treatment that is reproducible for the correct anatomic configuration —an ascending and aortic arch replacement, pericardial patch of the tracheal defect, and omental flap coverage. While this intervention is a large undertaking for a small defect, it is a safe and durable ...

Outcomes of expanded polytetrafluoroethylene pericardial membrane implantation in left ventricular assist device explantation and heart transplantation
ConclusionsIn patients undergoing LVAD explant-HT, there was a trend toward  reduced surgeon reported intraoperative bleeding with ePTFE placement. Despite qualitatively reported greater ease and speed of mediastinal dissection with ePTFE membrane placement, time to initiation of CPB did not differ, likely because surgeons remained cautious, allowing extra time for unantic ipated difficulties. Risk factors and mortality associated with permanent pacemaker after surgical or transcatheter aortic valve replacement: Early versus late implantation
ConclusionPPM placement after SAVR or TAVR procedures is not associated with increased mid-term  mortality. In-depth characterization of risk factors for early and late PPM implantation will require further analysis in the growing TAVR patient population. (Source: Journal of Cardiac Surgery)

Outcome of permanent pacemaker implantation in transcatheter or surgical aortic valve replacement: A still unsolved problem
AbstractDespite advances in technologies and clinical experience, conduction disorders, after transcatheter aortic valve replacement  (TAVR) or surgical aortic valve replacement (SAVR), represent the weak point of these procedures, requiring permanent pacemaker implantation (PPI) till 37.7% of patients in TAVR recipients. The role of PPI in TAVR and SAVR remains controversial in mid- and long-term outcomes. Indeed, many studies have been published with contradictory results, leaving doubts rather than certainties. ConclusionsPreoperative Impella support is not associated with increased short or long-term mortality but is associated with improved renal and hepatic function as well as total body perfusion before LVAD implantation. (Source: Journal of Cardiac Surgery)

Coronary artery occlusion flap: An unusual case of spontaneous coronary artery dissection
Conclusion: This patient's presentation of a dissection flap intermittently occluding the LMCA demonstrates an abnormal manifestation of SCAD. Increased awareness of this presentation is imperative to ensure future cases are correctly identified and managed. (Source: Journal of Cardiac Surgery)
