Cardiothoracic surgery leadership and learning are indispensable to each other
AbstractCardiothoracic surgery is facing a multitude of challenges in leadership and training on the global scale, these being a complex and aging patient population, shortage of cardiac surgeons, diminishing student interest and trainee enthusiasm, increasingly challenging training obstacles and work-life imbalances, suboptimal job prospects, reports of discrimination and bullying and lack of diversity as well as gap between innovation and technology, clinical application, and training of future surgeons. The survival of cardiac surgery hinges on the leadership attracting and retaining young surgeons into the specialty. M...

Impact of cerebrovascular accidents on lung transplant survival
ConclusionsPostoperative CVA following LT was associated with longer ICU stays, more ICU readmissions, longer length of stay, and fewer home discharges. Thirty day and long-term mortality were significantly higher in the CVA group. (Source: Journal of Cardiac Surgery)

Minocycline induced discoloration of the aortic intima and valve: A case report
ConclusionsGiven the rarity of this case, descriptions of cardiovascular ochronosis cases secondary to minocycline use are imperative in ensuring that it is on the differential diagnosis when identified by others in future cases. Additional care must be taken intraoperatively to ensure that the correct anatomy is identified as discoloration hinders visualization of the anatomy potentially resulting in unintentional consequences such as heart block or perivalvular leak as traditional visual cues for suture placement are distorted. Pediatric hydatid cyst with ventricular aneurysm and surgical treatment with dor procedure, case report
ConclusionIn conclusion, this case differs from other CHCs previously reported in the literature due to the advanced stage of the disease, atypical clinical presentation, and rare complications despite the young age of the case. The surgical method used in treating the patient makes the subject more interesting. (Source: Journal of Cardiac Surgery)

A new technique to adjust the length of artificial chordae during mitral anterior leaflet repair
ConclusionsThe short-term outcomes of the described technique are good with adequate leaflet coaptation in all treated patients. Long-term results are needed to assess the stability and durability of this repair technique. (Source: Journal of Cardiac Surgery)

Efficacy of chest X ‐rays after drain removal in adult and pediatric patients undergoing cardiac and thoracic surgery: A systematic review
ConclusionA routine chest X-ray following chest drain removal in adult and pediatric patients undergoing cardiac and thoracic surgery is not necessary. It can be omitted without compromising patient safety. Obtaining a chest X-ray should be clinically guided. Alternatively, bedside ultrasound can be used for the same purpose without the need for radiation exposure. (Source: Journal of Cardiac Surgery)

Coarctation of aorta combined with multiple aneurysms
We reported a case of a 53-year-old patient with coarctation of the aorta and multiple aneurysmatic changes on the aortic arch. Enhanced computed  tomography and reconstruction revealed significant coarctation and multiple aneurysmatic dilatations. The patient underwent stent implantation and was discharged with symptoms relieved. Follow-up examination progression of aneurysms, however, without symptoms. (Source: Journal of Cardiac Surgery)

Split right coronary artery and shepherd's crook course of direct origin of the conal artery: An unhitherto association in a case of rheumatic mitral stenosis
AbstractIn a 47-year-old lady, planned for redo percutaneous mitral commissurotomy for recurrent mitral valve stenosis, there was incidental detection of splitting of right coronary artery and direct origin and shepherd's crook course of the conal artery. Though these two anomalies have no hemodynamic significance, correct nomenclature and potential clinical implications have been described. (Source: Journal of Cardiac Surgery)

Ventricular septal defect complicating myocardial infarction: A case of delayed percutaneous transcatheter closure
AbstractA 57-year-old man suffered chest pain during the COVID-19 pandemic, but he delayed medical treatment due to fear of infection. After  4 months, symptoms of chest tightness and shortness of breath appeared. Electrocardiogram (ECG) revealed old myocardial infarction; color sonography and myocardial computed tomography revealed apical myocardial defect. He refused surgery and percutaneous transcatheter closure, and follow-up obser vation. After 22 months, the symptoms of chest tightness and shortness of breath aggravated. He recovered after percutaneous transcatheter closure, and was discharged. This case shows dela...

Mitral valve surgery via repeat median sternotomy versus right mini ‐thoracotomy: A systematic review and meta‐analysis of clinical outcomes
ConclusionRight MT is a safe alternative to the traditional re-sternotomy for patients who have had previous cardiac surgery. The approach offers a reduced length of hospital stay, ICU stay, and a lower risk of new-onset renal failure requiring dialysis. This review calls for robust trials in the field to further strengthen the evidence. (Source: Journal of Cardiac Surgery)

Preoperative pulmonary vascular resistance and neutrophil ‐to‐lymphocyte ratio predict the demand of extracorporeal membrane oxygenation after pulmonary endarterectomy
ConclusionsPVR and N/L ratio can correctly predict who is likely to receive ECMO treatment after PEA. Therefore, addressing the preoperative inflammatory status might be beneficial but further research is needed. (Source: Journal of Cardiac Surgery)
