Neutrophil to lymphocyte ratio predicts permanent pacemaker implantation in TAVR patients
ConclusionsIn this prospective study, higher NLR values on the day of TAVR day were associated with an increased PPI rate in patients undergoing TAVR with no previous CAs. It is advisable, being inflammation part of the process, to prolong the time of observation for all patients without CAs till at least 21 days not to miss any new CA necessitating PPI. Comparison of minimally invasive versus conventional thoracic aortic operations: Early and midterm results in a series of 624 patients
ConclusionsMini proximal aortic operations can be performed successfully without compromising the proven efficacy and safety of conventional access. In selected patients, MS was associated with very low mortality and morbidity rates. Additionally, MS demonstrated superior clinical outcomes as regards respiratory adverse events, when compared with FS. (Source: Journal of Cardiac Surgery)

Mini ‐sternotomy versus full sternotomy for isolated aortic valve replacement: A single‐center experience
ConclusionThis study examined the long-term outcomes of propensity-matched patients undergoing isolated AVR via FS or MS and identified no significant differences in outcomes over a 5-year  follow-up period. The decision for surgical approach is multifactorial and should be decided on a case-by-case basis taking into consideration patient anatomy, surgeon experience, and comfort, as well as patient preference. Arterial switch operation with coronary button extension and neo ‐pulmonary trunk realignment for a rare coronary pattern
AbstractCoronary anatomy is key for arterial switch operations as reimplantation for coronary artery patterns originating from the same sinus is often challenging. We experienced an extremely rare coronary artery anatomy case (Leiden convention: 1  R, 1LCx) and successfully performed an arterial switch operation with coronary button extension and neo-pulmonary trunk realignment maneuver. Right anterior minithoracotomy for redo aortic valve replacement after coronary artery bypass grafting with bilateral internal thoracic artery: A case report
AbstractA 63-year-old male, with a history of coronary artery bypass grafting using bilateral internal thoracic artery (ITA) grafts, underwent surgical aortic valve replacement (AVR). Avoiding the graft injury, we selected the right anterior minithoracotomy approach under cardiac arrest with systemic hyperkalemia with remaining bilateral ITA grafts open. Deep hypothermia was induced to obtain more reliable myocardial protection. We believe this strategy can be considered as a therapeutic option in patients requiring AVR but unsuitable for transcatheter AVR. (Source: Journal of Cardiac Surgery)

Risk factors for development of acute renal failure in 5077 coronary artery bypass grafting patients in the current era
ConclusionOur study identified prolonged CPB time, insertion of IABP, and low EF as independent risk factors for developing ARF requiring dialysis after CABG. The results suggest that shortening of CPB time and protection of cardiac function are important factors to prevent ARF and that special care should be taken to protect the renal function when the patient need insertion of IABP. Idiopathic focal fibrosing mediastinitis with unilateral pulmonary arterial and venous involvement
We report a case of a 1-year-old girl where computed tomography  (CT) angiography demonstrated presence of ill-defined heterogenous soft tissue thickening obliterating the mediastinal fat planes in right peri-hilar region and encasing the hilar right pulmonary artery and its segmental branches leading to marked narrowing with nonvisualization of the right-sided pulmonary veins. The case highlights the features of fibrosing mediastinitis as well as the role of CT angiography in the diagnosis and accurate depiction of the extent of involvement of the mediastinal vasculature and airways. Outcomes of acute type A aortic dissection during the COVID ‐19 pandemic: An analysis of the Society of Thoracic Surgeons Database
ConclusionsFor patients presenting to the hospital with ATAAD during the first surge of the pandemic, operative outcomes were similar to pre-pandemic despite a 30% reduction in volume. Out-of-hospital mortality from ATAAD during the pandemic remains unknown. Further understanding these findings will inform management of ATAAD during future pandemics. Myocardial recovery in a patient with dilated cardiomyopathy after short ‐term biventricular assist device support
We report a case of idiopathic dilated cardiomyopathy who went through a challenging course. The case was presented as acute heart failure syndrome, which rapidly declined into cardiogenic shock and cardiac arrest that required an extracorporeal membrane oxygenator, then biventricular assist device implantation for circulatory support. The course was complicated with severe gastrointestinal bleeding and multiorgan failure until achieving full cardiac and organ recovery. The left ventricle ejection fraction improved from 10% to 50% at discharge. (Source: Journal of Cardiac Surgery)

Acid citrate dextrose formula A versus unfractionated heparin for anticoagulation of salvaged red blood cells in cardiac surgery
AbstractRed blood cell salvage plays an important role in reducing the use of allogeneic blood transfusion during cardiac surgery. While there is consensus as to the benefit of employing cell salvage systems, there are no clear recommendations on the anticoagulant used for salvaged blood. In eight patients undergoing elective cardiac surgery at our university hospital's cardiovascular center, the authors describe hemodynamic effects of salvaged autologous blood transfusion when either unfractionated heparin or acid citrate dextrose formula A was used as the anticoagulant. Mean arterial pressure, heart rate, central venous ...
