Estimating the smallest worthwhile difference of antidepressants: a cross-sectional survey
Conclusions Our observed SWDs mean that the current 15% antidepressant benefit over no treatment was sufficient for one in three people to accept antidepressants given the burdens, but two in three expected greater treatment benefits. Implications While a minority may be satisfied with the best currently available antidepressants, more effective and/or less burdensome medications are needed, with more attention given to patient perspectives. Understanding and responding to the drivers of inequalities in mental health
BMJ Mental Health is delighted to announce a new section called Experience, Ethics, Equity that seeks submissions of primary research, systematic reviews, and perspectives and commentaries. We set out the case for such a section, and then give some guidance on article types and priority areas for research, practice and policy. Ethnic minorities and racialised groups are less likely to be represented in research and, with some exceptions, less commonly occupy higher management positions or have opportunities to influence policy and practice.1–4 There are ethnic and gender disparities in academic research environments ...

'Far Away from Home: adolescent inpatient admissions far from home, out of area or to adult wards: a national surveillance study
Conclusions At-distance and adult ward admissions for patients aged <18 remain an ongoing challenge for healthcare provision and have an impact on acute hospital resource use. Clinical implications Long waits in non-specialist settings increase pressure across the healthcare system, highlighting the need to improve local service provision and commissioning to reflect identified clinical needs. Understanding the protective effect of social support on depression symptomatology from a longitudinal network perspective
Conclusions Reductions in negative self-appraisal might function as a bridge between social support and other depressive symptoms, and, thus, it may have amplified the protective effect of social support. Men appear to benefit more from social support than women. Clinical implications Building community-based support networks to deliver practical support, and loneliness reduction components are critical for depression prevention interventions after stressful experiences. Socioeconomic status and severe mental disorders: a bidirectional multivariable Mendelian randomisation study
Conclusions Our results suggest a heterogeneous link pattern between SES and SMD. We found a negative bidirectional association between genetic liability to income and the risk of schizophrenia and depression. On the contrary, we found a positive bidirectional relationship of genetic liability to EA with schizophrenia and BD, which only becomes apparent after adjusting for income in the case of schizophrenia. Clinical implications These findings shed light on the directional mechanisms between social determinants and mental disorders and suggest that income and EA should be studied separately in relation to mental illness...

Genetically predicted causal associations between periodontitis and psychiatric disorders
Conclusions These results do not support bidirectional causal associations between genetically predicted periodontitis and 10 common psychiatric disorders. Potential confounders might contribute to the previously observed associations. Clinical implications Our findings might alleviate the concerns of patients with periodontitis or psychiatric disorders. However, further research was warranted to delve into the intricate relationship between dental health and mental illnesses. (Source: Evidence-Based Mental Health)

Relationship between self-stigma about alcohol dependence and severity of alcohol drinking and craving
Conclusions Higher levels of self-stigma were associated with more severe AUD, greater alcohol consumption, and more obsessive thoughts and compulsive behaviours related to alcohol. Clinical implications Our results suggest that potential interventions to reduce self-stigma may lead to improved quality of life and treatment outcomes for individuals with AUD. Comparative analysis of algorithm-guided treatment and predefined duration treatment programmes for depression: exploring cost-effectiveness using routine care data
Conclusions The algorithm-guided programme led to larger health gains than the predefined duration treatment programme, but it was considerably more expensive, and hence not cost-effective at current Dutch thresholds. Depending on the preferences and budgets available, each programme has its own benefits. Clinical implication This study provides valuable information to decision-makers for optimising treatment allocation and enhancing quality of care cost-effectively. ChatGPT and mental healthcare: balancing benefits with risks of harms
Against the global need for increased access to mental services, health organisations are looking to technological advances to improve the delivery of care and lower costs. Since November 2022, with the public launch of OpenAI’s ChatGPT, the field of generative artificial intelligence (AI) has received expanding attention. Although generative AI itself is not new, technical advances and the increased accessibility of large language models (LLMs) (eg, OpenAI’s GPT-4 and Google’s Bard) suggest use of these tools could be clinically significant. LLMs are an application of generative AI technology that can su...

Explaining paranoia: cognitive and social processes in the occurrence of extreme mistrust
Conclusions Multiple causes are likely to be involved in paranoia. Cognitive and social processes may explain a high degree of paranoia. Clinical implications Multiple clear targets for intervention to reduce paranoia are identified. (Source: Evidence-Based Mental Health)

Seasonal trends in antidepressant prescribing, depression, anxiety and self-harm in adolescents and young adults: an open cohort study using English primary care data
Conclusions There were higher rates of antidepressant prescribing and consultations for depression and anxiety at the start of the school year among adolescents. Clinical implications Support around mental health issues from general practitioners and others should be focused during autumn. (Source: Evidence-Based Mental Health)

Treatments for partial remission of major depressive disorder: a systematic review and meta-analysis
Conclusions Psychotherapy targeting partial remission may be effective in lowering depressive symptom severity and patients may potentially achieve full remission twice as likely. Yet, long-term and prophylactic effects are lacking. Given the risk of chronicity, more high-quality RCTs are needed. PROSPERO registration number CRD42020188451. Early parenting interventions to prevent internalising problems in children and adolescents: a global systematic review and network meta-analysis
Conclusions We found preliminary evidence that relationship-focused and mixed parenting interventions were effective in reducing child internalising problems, and the waitlist comparator increased internalising problems with implications for waiting times between referral and support. Considering the high risk of bias of most studies included, the findings from this meta-analysis should be interpreted with caution. PROSPERO registration number CRD42020172251. School-based Evaluation Advancing Response for Child Health (SEARCH): a mixed longitudinal cohort study from multifaceted perspectives in Jiangsu, China
Conclusions SEARCH will provide important insight into the developmental trajectory of mental well-being in Chinese children and adolescents. The study protocol does not simply focus on student self-report and incorporates caregiver and teacher viewpoints as well. It also collects objective indicators that may facilitate development of screening tools. Clinical implications We believe future study findings will guide the development and implementation of school-based mental healthcare initiatives to improve the well-being of children and adolescents.
