Choroidal Effusion following Intravitreal Brolucizumab Injection: A Case Report
This report describes a case of choroidal effusion after intravitreal brolucizumab injection for wet age-related macular degeneration (AMD). A 71-year-old Korean man with a history of wet AMD visited our clinic. On examination, the best-corrected distance visual acuity (BCVA) was 20/200 in the right eye. Fundus photography and optical coherence tomography showed wet AMD in the right eye. The patient showed no improvement while undergoing treatment with anti-vascular endothelial growth factor therapy (aflibercept, 6 times; ranibizumab, 5 times; and bevacizumab 3 times). We administered intravitreal brolucizumab injections i...

Transscleral Cyclophotocoagulation for the Treatment of Uncontrolled Glaucoma in a Boston Keratoprosthesis Type II Patient
Postoperative endoscopic cyclophotocoagulation (CPC) for the treatment of glaucoma in patients with Boston keratoprosthesis type II (BKPro II) was first described in 2017 by Poon et al. (Endoscopic cyclophotocoagulation for the treatment of glaucoma in Boston keratoprosthesis type ii patient. J Glaucoma. 2017 Apr;26(4):e146 –9). As we do not have this device, we present a case of transscleral CPC (TSCPC), in a BKPro II patient who had graft versus host disease and developed uncontrolled glaucoma. We dissected plane by plane to expose the bare sclera and performed the procedure as traditionally described. We concluded tha...

Viscoelastic-Induced Interlamellar Stromal Keratopathy (VISK) during Cataract Surgery in a Patient with a History of Laser in situ Keratomileusis
A 47-year-old patient status post uncomplicated bilateral LASIK treatment presented with interlamellar stromal keratopathy induced by viscoelastic material during cataract surgery, decreasing her vision to CF (count fingers) postoperatively. After recognition, the viscoelastic material was removed by interface irrigation. The keratopathy improved by postoperative day 1 and resolved by 6 weeks with best-corrected visual acuity being 20/30. We conclude that viscoelastic-induced stromal keratopathy requires correct recognition in order to undertake appropriate management.Case Rep Ophthalmol 2022;13:154 –157 (Source: Case Re...

A Case of Recurrent Fungal Keratitis Post-Amniotic Membrane Transplantation for Corneal Perforation
Here, we report a rare case of recurrent fungal keratitis (FK) post-amniotic membrane transplantation (AMT) for corneal perforation. A 75-year-old female who had undergone systemic 15 mg prednisolone administration for interstitial pneumonia developed FK in her left eye following treatment for herpetic epithelial keratitis at another clinic. The FK was effectively cured via an oral and local antifungal treatment. However, 1 year later, FK recurred in her left eye, and she was subsequently referred to our hospital since fungal infection had penetrated deep into the cornea. Upon examination, her best-corrected visual acuity ...

Hydrophilic Lens Opacification after Intravitreal Anti-VEGF Injections: A Case Series
Our objective in this retrospective case series was to report 4 cases of hydrophilic intraocular lens (IOL) opacifications after repeated intravitreal bevacizumab injections. This is a retrospective analysis of all the cases of IOL opacifications presenting to a tertiary referral ophthalmic center in Beirut between January 2013 and January 2019. Four cases were included in the study, of which one was treated for vitreal hemorrhage, the other for macular edema secondary to wet age-related macular edema, and the rest for macular edema secondary to diabetic retinopathy. The mean age of the patients was 71 years with a male pr...

Uveitis after COVID-19 Vaccination
This report describes a case of acute uveitis following COVID-19 vaccination. A 21-year-old patient was referred to our department for decreased visual acuity and injection of the left eye. Two days prior, the patient received her second dose of the severe acute respiratory syndrome coronavirus 2 vaccine (BNT162b2, Pfizer-BioNTech). On ocular examination, her left eye ’s best-corrected visual acuity was 20/250, and its intraocular pressure was 16 mm Hg on a noncontact tonometer. Hypopyon, 4+ cells, and flares were observed in the anterior chamber. The patient was diagnosed with acute uveitis. She received topical dexamet...

Successful Treatment of Epstein-Barr Virus-Induced Necrotizing Retinitis with Intravitreous Ganciclovir, Foscarnet, and Methotrexate
This is a report of an immunocompromised 49-year-old renal transplant patient with Epstein-Barr virus (EBV)-induced necrotizing retinitis (NR). The patient with NR underwent diagnostic vitrectomy. Polymerase chain reaction (PCR) testing of the vitreous fluid was positive for EBV (25,000 IU/mL) and negative for all other organisms. The patient was treated with intravitreous ganciclovir and foscarnet. After only mild clinical improvement in retinitis and an increased quantitative EBV PCR (69,000 IU/mL), intravitreous methotrexate was added to the aforementioned intravitreous antiviral injections. After eight rounds of gancic...

Bilateral Episcleral Corkscrew Vessels: Expedition into the Unknown – Case Report
In conclusion, our case highlights the systematic investigation necessary in cases of bilateral episcleral vessel engorgement and tortuosity and the possible differential diagnosis to be considered to rule out life-threatening causes of elevated episcle ral venous pressure. It is important for clinicians to be aware of IEEVP even in patients with atypical features that despite significative ocular hypertension had no glaucomatous damage.Case Rep Ophthalmol 2022;13:109 –115 (Source: Case Reports in Ophthalmology)

Reactivation of Herpes Zoster Keratitis Following Shingrix Vaccine
We present a case of herpes zoster keratitis reactivation shortly following the Shingrix vaccine. In our patient, reactivation of herpes zoster keratitis occurred a few weeks following the herpes zoster subunit (HZ/su) vaccine. The development of herpes zoster ophthalmicus following HZ/su is exceedingly rare, with only one prior reported case found in the literature. Reporting of this potential correlation is important in understanding the full risks of vaccines and can help elucidate the etiology of such responses.Case Rep Ophthalmol 2022;13:104 –108 (Source: Case Reports in Ophthalmology)

A Sudden Rise of Patients with Acute Macular Neuroretinopathy during the COVID-19 Pandemic
The aim of this paper is to inform on the surge of cases of acute macular neuroretinopathy (AMN) – a rare disease characterized by the sudden onset of acute scotomas caused by ischemia of the retinal capillary plexus – during the COVID-19 pandemic. In 2021, during the COVID-19 pandemic, a sudden rise in patients with AMN was observed in our clinic. In this paper, 4 cases from a hospital in the south of the Netherlands are reported, all of which could directly be linked to a COVID-19 infection or vaccination against the corona virus. A search for similar cases in the PubMed database produced thirteen relevant reports, w...

Recurrent Conjunctival Squamous Cell Carcinoma and Intraocular Tumor Extension after Topical Erythropoietin: A Case Report
Topical erythropoietin has been recently introduced for the treatment of avascular conjunctival and scleral lesions. Before this treatment can be routinely used, however, its safety profile and contraindications should be determined. Herein, we report a case of recurrent conjunctival squamous cell carcinoma (SCC) and intraocular tumor extension after treatment with topical erythropoietin for excisional biopsy-induced scleral necrosis. An 87-year-old man underwent excisional biopsy for a conjunctival leukoplakic mass. Histological examination showed a well-differentiated SCC on the postoperative day 10. All borders of the s...

Anaplastic Thyroid Carcinoma with Ocular Then Orbital Metastases
A 52-year-old Hispanic woman with a history of metastatic thyroid carcinoma presented with left eye degeneration and a choroidal mass without evidence of orbital disease on neuroimaging. She underwent enucleation for a blind, painful eye. Histopathology demonstrated choroidal metastasis of thyroid carcinoma with a well-circumscribed episcleral nodule, consistent with extraocular tumor extension. The interdisciplinary team recommended expectant management. Three months later, she developed periorbital edema and discomfort with poor ocular prosthesis fit. Neuroimaging revealed a mass in the anophthalmic socket that proved to...
