The availability, access, challenges and advancements in neurosurgical care in Africa: a mini review
Ann Med Surg (Lond). 2024 Feb 15;86(4):2011-2015. doi: 10.1097/MS9.0000000000001805. eCollection 2024 Apr.ABSTRACTThe availability and accessibility of neurosurgical care across Africa remains limited despite recent advancements. Overall, Africa accounts for 15% of the global neurosurgical disease burden but has access to less than 1% of neurosurgeons globally. While the number of neurosurgeons has increased in recent decades, huge workforce shortages remain, with the region facing the second-largest neurosurgical deficit. Access to adequate facilities and equipment is also lacking. Barriers like poverty, conflicts, and di...

A 30-year-old female with dermatomyositis without high elevation of muscle enzymes: a rare case report from Syria
CONCLUSION: Even if test findings are not conclusive, dermatomyositis should always be considered when muscular weakness manifests. It's important to distinguish the disorder from connective tissue diseases like lupus erythematosus. In fact, to correctly diagnose DM, if there are any doubts, a muscle biopsy is required.PMID:38576947 | PMC:PMC10990297 | DOI:10.1097/MS9.0000000000001682 (Source: Annals of Medicine)

Digital eye strain and its associated factors among radiology physicians in Pakistan: a cross-sectional survey using logistic regression analysis
CONCLUSION: With recent advances in the field of radiology in Pakistan, the frequency of developing DES among radiologists is increasing. Being a female, having long working hours, and having noticeable flickers on the digital screens were among the significant factors in developing DES among radiologists.PMID:38576948 | PMC:PMC10990352 | DOI:10.1097/MS9.0000000000001882 (Source: Annals of Medicine)

The risk of bone fractures in dementia patients receiving acetylcholinesterase inhibitors: a meta-analysis
CONCLUSION: Our study revealed no role for acetylcholinesterase inhibitors in increasing the risk of fracture compared with controls. Hence, based on our analysis, they might have a protective role against fracture when used for long periods considering their positive action on bone growth and development. Therefore, Acetylcholinesterase inhibitors could be considered a safe option for improving cognitive functions in elderly demented patients without carrying any additional risks.PMID:38576949 | PMC:PMC10990402 | DOI:10.1097/MS9.0000000000001721 (Source: Annals of Medicine)

Congenital single kidney in tuberous sclerosis complex (Bourneville disease)
CONCLUSION: This case report highlights Bournevile disease in a patient with a congenital single kidney, a rare finding that further complicates the disease. It emphasises the importance of recognising and managing this condition to ensure appropriate care for affected individuals.PMID:38576950 | PMC:PMC10990388 | DOI:10.1097/MS9.0000000000001834 (Source: Annals of Medicine)

Intracranial osteochondroma arising from the posterior clinoid process: a rare case report with diagnostic challenges and comprehensive literature review
CONCLUSION: Intracranial osteochondroma poses diagnostic challenges, especially near bone. Tailored surgical approaches are vital, with complete resection yielding good outcomes for convexity and falx cases. Close follow-up is crucial for monitoring recurrences and complications.PMID:38576951 | PMC:PMC10990327 | DOI:10.1097/MS9.0000000000001855 (Source: Annals of Medicine)

Cholangioscopy with Spyglass DS using percutaneous transhepatic cholangiography access: a retrospective cohort study
CONCLUSION: S-PTCS is a safe modality, with high success rates, high predictive values, and a low rate of complications. This study suggests that S-PTCS is an alternative to conventional methods in patients with indeterminate biliary strictures where conventional methods were unfeasible.PMID:38576952 | PMC:PMC10990305 | DOI:10.1097/MS9.0000000000001840 (Source: Annals of Medicine)

Unveiling the complexity of right middle lobe syndrome: a case series highlighting the association with asthma
CONCLUSION: These cases exhibited obstructive MLS with and without asthma. Accurate diagnosis is challenging, requiring imaging techniques. MLS has clinical implications, particularly in asthma patients. Future studies should focus on understanding the aetiology of non-obstructive MLS.PMID:38576953 | PMC:PMC10990357 | DOI:10.1097/MS9.0000000000001848 (Source: Annals of Medicine)

Intraoperative electrocorticography-guided resection of the epileptogenic zone in an unusual porencephalic cyst: case report and literature review
CONCLUSION: Our findings emphasize the need to consider aetiology during interpretation of ioECoG to better define the electrophysiological border between the normal and epileptogenic brain, aiding in achieving a better surgical outcome.PMID:38576954 | PMC:PMC10990310 | DOI:10.1097/MS9.0000000000001871 (Source: Annals of Medicine)

Challenges and lessons learnt in the management of an HIV-exposed neonate with gastroschisis in a resource-limited setting: case report
CONCLUSION: This case of gastroschisis shows how the diagnosis and management of neonates born with major congenital structural abnormalities in resource-limited settings is still desirable due to lack of sophisticated medical care services to assist in early detection during pregnancy and early surgical intervention at birth to prevent associated mortality. The authors discuss the lessons learnt and provide recommendations for improvement in the care of neonates born with abdominal wall defects and other congenital birth defects.PMID:38576955 | PMC:PMC10990413 | DOI:10.1097/MS9.0000000000001924 (Source: Annals of Medicine)

Awake brain mapping by direct cortical stimulation; technical note to get higher resection rate and low morbidity in low-grade glioma patients
CONCLUSION: Functional realignment in shifted brain and edema can be seen while doing cortical and subcortical stimulation. Most of the stimulation from low to high for language mapping may vary from patient to patient. For safe removal of low-grade glioma a steep learning curve is needed to find out the negative areas, though the authors emphasize positive mapping of areas to secure the maximum eloquence.PMID:38576956 | PMC:PMC10990364 | DOI:10.1097/MS9.0000000000001837 (Source: Annals of Medicine)

Chorea as the only presenting clinical feature of rheumatic fever: a case report
CONCLUSION: Any child presenting with a movement disorder should also be considered for SC, necessitating additional testing, including a cardiovascular assessment. It needs to be distinguished from other causes of movement disorders as well as psychiatric conditions. Treatment is necessary for moderate to severe chorea that interfere with daily activities. Compliance with subsequent antibiotic prophylaxis is essential for avoiding future cardiac complications.PMID:38576957 | PMC:PMC10990359 | DOI:10.1097/MS9.0000000000001798 (Source: Annals of Medicine)

This study was designed to determine the analgesic effect of caudal Dexmedetomidine with Ropivacaine in paediatric genitourinary infraumbilical surgeries. Postoperative analgesic effects of caudal Ropivacaine with or without Dexmedetomidine in paediatric genitourinary infraumbilical were evaluated. This study was a prospective, interventional, comparative study conducted after ethical approval from the institute. Informed expressed consent was taken from each patient's guardians. The sample size was calculated to be 31 in each group. The two groups were randomly assigned and the intervention involved caudal epidural inject...
