The Virtual CTSA Visiting Scholar Program to Support Early-Stage Clinical and Translational Researchers: Implementation and Outcomes
This article describes the design and short-term outcomes of the virtual CTSA Visiting Scholar Program. In 2020, a working group designed core program elements and developed an application and selection process. Anonymized surveys were sent to scholars post visit and to scholars and program directors 6 months post visit to evaluate their experience and solicit suggestions for improvements. Between November 2020 and May 2021, 56 KL2 scholars and 27 hubs participated. Forty-five (80.4%) participating scholars responded to the initial survey. Nearly all scholars (44, 97.7%) agreed their experience was valuable. All respondent...

Early Outcomes of a New NIH Program to Support Research in Residency
This article reports on the early outcomes of the recipients of the initial round of R38 awards, granted in 2018. Early positive outcomes include increases in the reported likelihood of resident–investigators pursuing physician–investigator careers, greater reported clarity in resident–investigators’ research directions, the commitment of additional institutional resources to support the R38-awarded programs, and the approval of resident–investigators as having met training requirements for certification by multiple medical boards. Beyond Humility: Empowering Minoritized Learners Through Culturally Reflective Medicine
In the wake of the Black Lives Matter and other antiracism justice movements, medical education is evolving to incorporate health equity principles for all medical students and residents, while also increasing institutional diversity recruitment of minoritized physicians. As the demographics of U.S. medical trainees shift to reflect a rapidly evolving patient population, the prevalence of culturally concordant patient–physician visits, where patients share elements of language, religion, customs, and identities with their physician, is also expected to increase. However, existing antiracist curricula are standardized to ...

Toward Trauma-Informed Pedagogy: An Intersectional Analysis of Pimping in Medical Education
Increased awareness of the role of power and privilege in medical education demands reevaluation and critical reflection on the role of pimping, both in name and practice. The authors use an intersectional approach to explore the ways in which the term pimping is rooted in sexist, racist, and classist stereotypes. In using this term to describe a pedagogical approach rooted in hierarchical power and stark knowledge differentials, the medical community intentionally or unintentionally reinforces racist stereotypes, acts of carceral violence, gender-based violence, and harmful ways of referring to sex work. The authors sugge...

A Call to Action From the California Consortium for the Assessment of Clinical Competence: Making the Case for Regional Collaboration
The discontinuation of the United States Medical Licensing Examination Step 2 Clinical Skills Examination emphasizes the need for other reliable standardized assessments of medical student clinical skills. For 30 years, the California Consortium for the Assessment of Clinical Competence (CCACC) has collaborated in the development of clinical skills assessments and has become a valuable resource for clinicians, standardized patient educators, psychometricians, and medical educators. There are many merits to strong multi-institutional partnerships, including the integration of data across multiple schools to provide feedback...

The Purpose, Design, and Promise of Medical Education Research Labs
Medical education researchers are often subject to challenges that include lack of funding, collaborators, study subjects, and departmental support. The construct of a research lab provides a framework that can be employed to overcome these challenges and effectively support the work of medical education researchers; however, labs are relatively uncommon in the medical education field. Using case examples, the authors describe the organization and mission of medical education research labs contrasted with those of larger research team configurations, such as research centers, collaboratives, and networks. They discuss seve...

Rethinking the Physician–Scientist Pathway
Physician–scientists have the potential to generate fundamental as well as translational breakthroughs. But many trainees who intend to pursue a hybrid career in research and patient care ultimately leave one or the other behind. In this Invited Commentary, the authors draw from their experience as early-career physician–scientists to frame physician–scientist training as having 2 phases: first, learning to think like a physician–scientist; second, learning to act like a physician–scientist. These phases roughly correspond to (1) clinical training (from medical school through residency or fellowship) that incorpo...

Improving the Health of Rural Communities Through Academic–Community Partnerships and Interprofessional Health Care and Training Models
Health disparities between rural and urban areas are widening at a time when urban health care systems are increasingly buying rural hospitals to gain market share. New payment models, shifting from fee-for-service to value-based care, are gaining traction, creating incentives for health care systems to manage the social risk factors that increase health care utilization and costs. Health system consolidation and value-based care are increasingly linking the success of urban health care systems to rural communities. Yet, despite the natural ecosystem rural communities provide for interprofessional learning and collaborativ...

Rural Graduate Medical Education: Choosing the Road “Less Traveled By”
Despite active efforts to improve access to health care for people who live in rural communities, the need for developing a physician workforce that is committed to rural practice, and with the professional and personal skills that will best fit with the needs of the rural community, is only increasing over time. Supporting and solidifying the rural graduate medical education landscape must be a crucial piece of any plan to address gaps in rural health care. Embracing creative solutions that address the most important barriers to this development has high potential for meeting the health care needs of rural communities, wi...
