Why is it so hard to speak out about patient safety?
In the previous episodes of Doctor Informed, we've heard why it's so important to talk about patient safety concerns, and some of the mechanisms that allow hospital staff to raise them, but knowing why and how doesn't always make it easier to speak out. In this episode we're exploring the concept of a voiceable concern – identifying what counts as a concern, and what counts as an occasion for voice by an individual, is not a straightforward matter of applying objective criteria- for example how do you tell if you're witnessing poor practice, or just something that lies outside your area of understanding? Are corticosteroids (anti-inflammatory medicines) given orally or by injection an effective treatment for people with COVID-19?
Cochrane has produced several reviews to help decision makers respond to the COVID-19 pandemic. In August 2021, we published the first version of a living systematic review on systemic corticosteroids and, in this podcast, the first authors, Mirko Griesel of the University of Leipzig Medical Center and Carina Wagner of the University Hospital in Cologne, Germany, talk about the evidence they ’ve found and the potential effects of these drugs. SCCM Pod-449 FNP in ICU
Family nurse practitioners (FNPs) who do not have acute care certification may be recruited to work in ICUs that lack enough acute care nurses, which is a challenge for both the ICU and the FNP. When the FNP moves on to another institution, that institution may not consider the FNP's ICU experience to be an adequate qualification. Host Diane C. McLaughlin, DNP, AGACNP-BC, CCRN, is joined by Christian Santos, MSN, FNP-BC, and Mariah Rose, ARNP, both nurse practitioners at Mayo Clinic in Jacksonville, Florida, to discuss the difficulties of FNPs who need acute care certification to work in ICUs. Christian Santos, MSN, FNP-BC...

CAR T-cell therapy for people with diffuse large B-cell lymphoma which returns after treatment or no longer responds to treatment
Diffuse large B-cell lymphoma is a fast-growing cancer of the lymphatic system. A new Cochrane Review from September 2021 looks at the evidence on the effects of one treatment, chimeric antigen receptor (CAR) T-cell therapy for people whose disease relapses or does not respond to other treatments. We asked the lead author, Moritz Ernst from the Faculty of Medicine and University Hospital Cologne in Germany to tell us what they found. (Source: Podcasts from The Cochrane Library)
