The practice of medicine is a marathon
We physicians often feel as though we are running up Heartbreak Hill. For many of us, the spring in our step just ain ’t what it used to be. Pictured above is Ethiopian runner Worknesh Negefa, the fourth-fastest female marathoner in history and the winner of the 2019 Boston Marathon.  We all understand why Boston 2020 […]Find jobs at  Careers by KevinMD.com.  Search thousands of physician, PA, NP, and CRNA jobs now.  Learn more. Answer to Case 629
Answer toParasite Case of the Week 629: hookworm ova - eitherAncylostoma duodenaleorNecator americanus. As stated well by Sam, "Finally! An end to the " is this hookworm? " trilogy. Haha 😁 " You can see a couple of good examples of hookworm egg mimics in my last two cases. He and TheOracle also noted that the second image had Charcot-Leyden crystals, a breakdown product of eosinophils:Idzi commented that "If you would ask for the " most-likely " identification, I ' d dare guessing " Ancylostoma duodenale " - not per s é due to the fact that the patient is from the African continent (as migration and trav...

Sunday Sermonette: More rules
Deuteronomy is probably the most tedious book of the Bible, certainly of the Torah. It ' s basically a laundry list of rules, interspersed with references to events in Exodus through Numbers, though often inaccurate. The rules range from some which seem morally salutary to modern readers, to strange in modern context, to arbitrary and ridiculous, to depraved. Chapter 24 is mostly in the contextually strange category, I would say. Fortunately we ' re getting near the end. There ' s more action in Joshua, for better or (mostly) for worse. 24 “When a man takes a wife and marries her, if then she finds no favor in ...

Few Immigrants among Capitol Insurrectionists
Alex NowrastehAs of February 1, 2021, a  total of 217 people have been arrested or charged with participating in the January 6, 2021 Capitol insurrection. Many more people will be arrested and charged in the coming months, but it’s worth looking at the demographics of this group to better understand what happened.Newresearch by professors Robert A. Pape and Keven Ruby present the age, employment status, and other demographic characteristics of the mob that attacked the Capitol. However, Pape and Ruby left out one key demographic variable: nativity. How many of the rioters were foreign ‐born? To fill this gap, my int...

Some things you may want to know about.  Apologies for the long gap since the last list.  Maybe monthly is more realistic, or maybe I need to be more organised.First,COVID, a systematic review by three Ethiopian authors on The effect of coronavirus infection (SARS-CoV-2, MERS-CoV, and SARS-CoV) during pregnancy and the possibility of vertical maternal-fetal transmission: a systematic review and meta-analysis published in the open access European Journal of Medical Research. The Guardian reports thatmothers are needlessly separated from their babies after birth, and has articles about pregn...

Sunday Sermonette: Identity crisis
Numbers 12 is puzzling in several ways, which the Midrash (a rabbinical commentary on the Torah) attempts to explain. The first puzzle is that Miriam and Aaron complain about Moses ' s " Cushite " wife. KJV has " Ethiopian " which is close enough. Cushite refers to people from the area south of Egypt, including what is today Sudan, Somalia, and Ethiopia. But when Moses fled after killing the Egyptian he sojourned among the Midianites, which is the identity of his wife Zipporah and her father. We don ' t know what direction he fled, but we do know that the Midianites are from Palestine. In one version of the abduction...

Teaching As a Human Trait
What did people have to talk about when language was new? They had been getting along fine without words, and suddenly they had a few, but what was there to say?Donald M. Morrison has   written a book (The Coevolution of Language, Teaching, and Civil Discourse among Humans) that proposes language got up and running as a teaching system. Speculation about teaching is common, but usually limited to teaching how to make stone tools. Opinions are mixed as to whether language was necessary to teach how to make the early tools, especially Oldowan tools. Showing without talking might well have been enough to teach how to make th...

Details of 155 Immigration Detainers for U.S. Citizens
David J. BierImmigration and Customs Enforcement (ICE) routinely requests that local law enforcement detain U.S. citizens to allow it to pick them up. ICE ’s recordslist 3,158 U.S. citizens as targets of ICE detainers from October 2002 to September 2019. Another 1.6 percent of actual ICEarrests through Secure Communities —the targeting system that ICE uses to issue detainers—were U.S. citizens from October 2008 to April 2011—or 3,627 citizens. Immigration courts—again with incomplete records—show 2,549 removal proceedings terminated in the favor of U.S. citizens from 2002 to June 9, 2017.These totals undercount...

Do Elizabeth Warren and Bernie Sanders Really Care About Wealth Inequality?
Ryan BourneSenator Bernie Sanders has called levels of U.S. wealth inequality“outrageous,” “grotesque” and “immoral.” Democratic presidential candidate Elizabeth Warren is pushing for a wealth tax to curb what she describes as “runaway wealth concentration.” Yet despite their rhetoric, it’s not clear, deep down, whether either really cares about wealth inequality per se or believes that reducing it should be an overriding public policy goal.To see why, consider this. Every year,Credit Suisse calculates a wealth “Gini coefficient” for major countries, indicating their level of wealth inequality in a s...
