Ultrahigh-Resolution Photon-counting CT Useful Prior to Transcatheter Aortic Valve Replacement
Assessment of coronary anatomy prior to Transcatheter Aortic Valve Replacement has great prognostic significance. Invasive coronary angiography is done prior to TAVR though CT coronary angiography has a role in ruling out coronary artery disease in those with low or intermediate pretest probabilities. Accurate interpretation of coronary arteries in those planned for TAVR has challenges because of high occurrence of abnormal heart rhythm, contraindications for beta blockers and nitroglycerine administraton. Still important is the blooming artifacts on CT coronary angiography in those with coronary stents or severe calcific...

Localising WPW : Take it EASY always
If some body is struggling with same problem, say for over three decades , there is something seriously wrong with the way we deal with the problem or there is some problem with the problem itself . How do you localise accessory pathway in WPW syndrome from 12 lead ECG ? is one such entity, This question is asked exclusively in cardiology board exams. Now a 2023 paper from EUROPACE claims, it has come out with a simple algorithm bettering all the previous ones. Please check it for yourself. One thing we can say with conviction is most of these embryological remnant pathways are posteriorly or laterally dragg...

The anatomy lesson of Dr Tulp - De anatomische les van Dr Nicolaes Tulp
I wrote this ages ago but for some reason never published it.  But here it is!This painting by Rembrandt is in the Mauritshuis in Den Haag.  Last year on holiday I visited the gallery to see The Girl with the Pearl Earring (who in Dutch is just " The girl with the pearl " , although it doesn ' t rhyme in Dutch), butDe anatomische les van Dr Nicolaes Tulp is there too.  This was Rembrandt ' s first commission on moving to Amsterdam, commissioned by the Surgeon ' s Guild.Tulp gave annual anatomy lectures, open to the public (on payment).   The paper being held by one of the surgeons pr...
