21-Year-Old Man With Fever, Night Sweats, Productive Cough, and Diarrhea

A 21-year-old White man with Gilbert syndrome presented to the emergency department of Mayo Clinic in Arizona in August 2019 with approximately 10 days of cough and 5 days of progressively severe fever, night sweats, vomiting, and diarrhea. He also noted antecedent intermittent nausea, vomiting, diarrhea, and abdominal cramping over the previous 6 months that improved with dietary modification. Review of systems was also positive for right-sided pain in the lower thorax and upper abdomen. The patient had no notable medical or surgical history besides cochlear implant placement for congenital deafness.
