Systolic Color Doppler Flow Into The Aorta Through The Left Main Coronary Artery: What Is The Diagnosis?

A 5-year-old-boy (weight, 17.4 kg; height, 120 cm; body surface area, 1.02m2) with pulmonary atresia, intact ventricular septum, bipartite hypoplastic right ventricle and tricuspid valve hypoplasia presented to the author's institution for pre-Fontan evaluation. The child had under gone a bi-directional Glenn procedure at one-year of age. The electrocardiogram showed a sinus rhythm, left axis deviation, and ‘ST’ depression in V1-V3 leads. The origin of the left main coronary artery was dilated in a parasternal short axis view of the aorta by transthoracic echocardiography as well as in mid-esophageal aortic short axis view by transesophageal echocardiography [Figure 1 A&B].
