Successful transplantation of high-risk cardiac allografts from DCD donors following ex-vivo coronary angiography

Despite increased use of marginal donor hearts including the utilization of donation after circulatory death (DCD) allografts, there remains a global paucity of donors to match the burgeoning waiting lists. The pressures of an aging population have led to the necessary acceptance of older donors with higher risk profiles for coronary artery disease (CAD).1 A lack of facilities or time constraints on the donor workup may preclude coronary angiography on such donors. In addition, in the DCD setting, coronary angiography is often refused on ethical and/or legal grounds.
