We report three patients with history of fever and different clinical signs. During the height of the pandemic in Italy (March to May 2020), these patients underwent chest computed tomography (CT) scans that showed lung alterations typical of COVID-19 with multiple negative RT-PCR tests and positive serology for SARS-CoV-2.

ConclusionIt seems unnecessary to perform chest imaging in children to diagnose COVID-19. Chest radiography can be used in symptomatic children to assess airway infection or pneumonia. CT should be reserved for when there is clinical concern to assess for possible complications, especially in children with coexisting medical conditions.

Coronavirus Disease 2019 (COVID-19) has caused a large number of related deaths and multiple healthcare associated outbreaks [1,2,3,4]. Investigators have summarized the clinical characteristics of patients with initial onset [3,4], in which, some had atypical clinical manifestations, e.g., severe patients merely had moderate or low fever or even no fever [5], while some mild patients may not have pneumonia and only had low fever or mild fatigue [5,6]. One report revealed that no radiographic or CT abnormalities were found in 17.9% of non-severe patients and in 2.9% of severe patients [6].

Conclusions: On the whole, the children were previously healthy and are more than 1 year old. Respiratory symptoms were the leading cause of PICU admission, making respiratory support the principal therapy. Patients requiring mechanical ventilation showed deterioration on the first day of admission. These children seemed to require close monitoring, and multicenter studies are necessary.
