Labetalol Infusion Attenuates Paradoxical Hypertension and Decreases Plasma Renin Activity After Repair of Coarctation of the Aorta in Children

SEVERE hypertension after repair of coarctation of the aorta in children is a common clinical problem that requires pharmacologic treatment in the majority of patients.1-3 Such hypertension is termed “paradoxical hypertension” because it occurs after the aortic obstruction has been surgically relieved. The following 2 phases of the hypertension have been described: the first, a marked elevation of predominantly systolic pressure occurring within the first 12- to- 48 hours after surgery and t he second, an elevation of systolic and predominantly diastolic pressure occurring at 2- to- 4 days after surgery.
