We report the successful management of sedation supported by supplemental oxygenation using a high-flow nasal cannula1 (HFNC) in a patient with compromised respiration undergoing percutaneous balloon aortic valvuloplasty under continuous monitoring with transesophageal echocardiography (TEE). An 85-year-old, 49 kg man with a history of severe aortic valve stenosis, chronic heart failure, pulmonary hypertension, pneumorrhagia, and chronic obstructive pulmonary disease on home oxygen therapy was admitted for acute exacerbation of chronic obstructive pulmonary disease and heart failure triggered by pneumonia.
