Bacteria: back pain, leg pain and Modic sign —a surgical multicentre comparative study

ConclusionsCutibacterium acnes found in discs and vertebrae during surgery for disc herniation in adults with degenerated discs may be caused by contamination, as findings in this group were similar to findings in a control group of young patients with scoliosis and non-degenerated discs. Furthermore, such findings were almost always combined with bacterial findings on the skin and/or in the wound. There was no association between preoperative Modic changes and bacterial findings. Antibiotic treatment of lumbar disc herniation with sciatica and/or low back pain, without signs of clinical discitis/spondylitis, should be seriously questioned.Graphic abstractThese slides can be retrieved under Electronic Supplementary Material.

Renal angiomyolipoma (AML) is the most common benign solid renal neoplasm seen in daily clinical practice, with an estimated prevalence of 0.2% to 0.6%.1 It is a triphasic mesenchymal neoplasm composed of varying amounts of dysmorphic vasculature, smooth muscle, and mature adipocytes. Pathologically, AML is now considered among the family of perivascular epithelioid cell tumors. Approximately 80% of cases are sporadic, with mean age at presentation being 43 years and most cases identified at fourth to sixth decades of life. There is also a strong female predilection, with female-to-male ratio of 4:1 in sporadic cases.1 Twe...

Abstract Acromegaly is a debilitating and disfiguring chronic disease, which occurs in both sexes at any age, associated with multiple comorbidities and increased mortality. It is typically caused by a GH-secreting pituitary adenoma that promotes exposure of body tissues to increased concentrations of GH and IGF-I. The diagnosis of acromegaly is still made very late in a substantial number of patients when the disease is already in advanced stages. An epidemiological study from Sweden has elegantly demonstrated that the longer the diagnostic delay in acromegaly, the higher the number of comorbidities. Moreover, ab...

This study was conducted to compare the anesthetic and analgesic efficacy of bupivacaine with other local anesthetic agents routinely used for mandibular third molar surgery.Method and materials: Four electronic databases (PubMed, Scopus, Cochrane, and Web of Science) were explored to isolate randomized controlled trials up to 10 February 2019. The anesthetic and analgesic efficacies were assessed using six evaluation outcomes: onset of anesthesia, success of anesthesia, duration of anesthesia, duration of analgesia, pain score on the fourth postoperative hour, and number of analgesics consumed. Stata software (version 13,...

Authors: Piazza MJ, Gonzales-Zamora JA Abstract Neisseria gonorrhoeae is an uncommon present-day cause of septic arthritis. It is generally seen in the younger patient population and is often difficult to isolate in the lab. Blood cultures performed as routine work are usually negative, and when positive tend to be seen in the classic form of disseminated gonococcal infection. Here we report a case of acute septic monoarthritis, associated with N. gonorrhoea bacteraemia, in a 67-year-old male patient with multiple chronic comorbidities, who presented with acute pain and swelling at his left elbow, and no associated...
