Sudden onset of temporomandibular joint swelling and limited mouth opening: a rare case of ganglion cyst

Tal, Berg / Boaz, FrenkelPage 816 - 820Preauricular swelling followed by limited mouth opening can be a challenging diagnosis. All anatomical structures and related pathologies should be examined. A 41-year-old woman presented to the Emergency Room complaining of limited mouth opening, severe pain that was worse on chewing, and swelling over her right temporomandibular joint (TMJ) that had started 3 days previously. Physical examination revealed localized swelling and redness over her right TMJ. Her right masticatory muscles were very painful and tender on palpation. Mouth opening was slightly limited. A computed tomography (CT) scan demonstrated a collection adjacent to her right TMJ, and reactive arthritis was diagnosed. Aspiration and arthrocentesis were performed, leading to immediate improvement. When symptoms returned 6 months later, magnetic resonance imaging (MRI) demonstrated a cystic lesion adjacent to the TMJ. Under general anesthesia an excisional biopsy was performed, and the pathologic examination revealed fibrous connective tissue with pseudocyst-like formation compatible with ganglion cyst. This case report demonstrates that a CT scan might lead to an incorrect diagnosis, whereas MRI scanning led to the correct diagnosis, thereby emphasizing the importance of choosing the right imaging modality. When a pathologic process of the TMJ is suspected, MRI is the gold standard imaging modality to diagnose the exact TMJ pathology. Complete surgical excision of ganglio...

Short-eared Owl at Burwell Fen, photographed mid-January One evening in late November, I was once again, hoping to catch sight of the Starling murmurations that occur over the Broad Lane balancing pond. As mentioned in a previous, issue the local Starlings and their continental counterparts will often roost in the reed bed there, last winter there were literally thousands. At the time of writing, just a few hundred are roosting, but that can change on a wind as arrivals from Europe turn up when the weather changes. Anyway, reader Alison waved as she passed the pond on her dog walk. I later heard that she’d seen a scu...

Abstract The concentration of propofol in blood is an important indicator for anesthesiologists to monitor and regulate the anesthesia depth of patients during surgery. Herein, a negative photoionization ion mobility spectrometry with acetone as the dopant was developed for rapid and direct determination of intraoperative blood propofol concentration in the operating theatre. High concentration of acetone molecules in the carrier gas was used not only to enhance neutral desorption and release free propofol molecules from the whole blood, but also to increase the intensity of reactant O2- and reduce the amount of n...

Abstract In this work, a selective silica-based molecular imprinted solid-phase microextraction (SPME) sorbent was prepared through the sol-gel process. Difenoconazole was used as a template to prepare imprinted materials on the surface of mesoporous silica. The SPME fiber followed by gas chromatography-electron capture detection was applied for the extraction and determination of difenoconazole. Fourier transform infrared spectrometry, field emission scanning electron microscopy, X-ray diffraction, and thermal gravimetry were used to characterize the imprinted SPME fiber. Two different procedures were presented t...
