Treatment outcomes of drug-resistant tuberculosis in the Netherlands, 2005 –2015

ConclusionsIn recent years, we identified a low incidence of DR-TB as well as the poor outcome of DR-TB treatment. The majority of cases were primary drug-resistant and foreign-born. To further improve treatment outcome, special attention should be given to the high-risk DR-TB patients.

Conclusions: Applying a predictive model with clinical and laboratory indices can facilitate the differential diagnosis of TPE from SMPPE in children, which seems helpful when a microbiologic or histologic diagnosis of pleural tuberculosis could not be established.

In this review, we report the available data regarding efficacy, safety and pharmacokinetics of colistin in the treatment of multidrug-resistant Gram-negative bacteria in neonates and infants. Seventeen clinical studies, involving 312 patients, and 3 pharmacokinetics studies were identified. Diagnosis of pediatric tuberculosis is notoriously difficult. We investigated the additional yield of blood culture in hospitalized children in Vietnam. Among 554 enrolled clinically suspected patients, an additional 6 cases were diagnosed, while the incremental cost per case was USD500. Addition of blood culture is therefore not recommended for our total patient population, but may be considered in specific groups.

The infectious risk of ERCP has historically been ascribed to biliary stasis and obstructive physiology from stones, strictures, or stents. Recent outbreaks, many of them from multidrug-resistant organisms (MDROs), have been attributed to the infectious carriage of the duodenoscope itself: so-called exogenous infection. This has drawn considerable attention and research efforts intended to better characterize the contributory risk of each of the following factors: instrument design, reprocessing measures (eg, manual and automated cleaning), surveillance strategies for detecting instrument contamination, and other quality metrics.

We present the case of a 30-year-old Bissau-Guinean man with HIV-2 infection known for 16 years, irregularly treated, admitted with two weeks of fever, diarrhoea and cutaneous lesions. ConclusionsCompared to linezolid treatment, daptomycin treatment showed comparable clinical and microbiological outcomes but a lower incidence of thrombocytopenia. Because of the dose-dependent effect observed, high-dose daptomycin should be considered for patients with VRE bacteraemia.
